Objective:To explore the diagnostic value of the dual-layer spectral detector CT for the distinguishing of parotid tumors.Methods:In this retrospective study,92 patients with histopathologically confirmed parotid tumors who underwent contrast-enhanced CT scan on the dual-layer spectral detector CT were analyzed.The morphological and parameters of dual-layer spectral detector CT were analyzed: The iodine concentration(IC),normalized iodine concentration(NIC),slope of the spectral Hounsfield unit curve(λHU)and effective atomic number(Z)in the dual-phase.quantitative data was used one-way ANOVA or Kruskal-Wallis test,Pearson chi-square test or Fisher exact test was used for classification data.statistical significance was set at P<0.05.Results:One hundred and seventeen tumors(92 patients)were confirmed histopathological,comprising of 38 pleomorphic adenomas(PAs),55 Warthin tumors(WTs)(30 patients)and 16 malignant tumors(MTs).IC,NIC,λHU and Z of arterial phase were statistically significant among three groups(all P<0.001).There was no significant difference in IC,NIC,λHU and Z of venous phase(P<0.05).groups PA and WT,groups PA and MT,and groups WT and MT were statistically significant(P<0.05).The NIC of arterial showed the best performance for distinguishing PA and MT,PA and WT,WT and MT,the AUC were 0.807(95%CI:0.684-0.929),0.994(95%CI: 0.985-1),0.894(95%CI: 0.822-0.966),respectively.The optimal NIC of arterial thresholds for distinguishing PA and MT was 0.08,sensitivity and specificity were 78.3%,72.4%,The optimal NIC of arterial thresholds for distinguishing PA and WT was 0.12,sensitivity and specificity were 96.6%,94.5%,The optimal NIC of arterial for distinguishing WT and MT was 0.14,and sensitivity and specificity were 78.3%,83.6%.Conclusion:Quantitative parameters of double-layer detector spectral CT were helpful for qualitative diagnosis of parotid tumors and the NIC of arterial showed the best performance for parotid tumors.
