Objective:By enzyme-linked immunosorbent assay(ELISA),the levels of serum autophagy-related proteins Beclin1 and LC3 in patients with Metabolic dysfunction-associated fatty liver disease(MAFLD)were detected,and by microplate method,the levels of serum lipoprotein lipase(LPL)and hepatic lipase(HL)were detected,and the correlation between the changes of Beclin1、LC3、LPL and HL with MAFLD was explored,to investigate their possible mechanism of development in patients with MAFLD and diagnostic value in patients with MAFLD,so as to provide a new direction for the early noninvasive serological diagnosis and treatment targets of MAFLD.Methods:68 patients with a clear diagnosis of MAFLD from October 2020 to October 2021 in our physical examination center and inpatient department were randomly selected as the research group(MAFLD group),and they were further divided into 3 groups using abdominal ultrasound results:mild,moderate and severe MAFLD group.At the same time,66 healthy volunteers who were randomly examined at the health examination center of our hospital were randomly selected as the control group(NC group).General information such as subject name,gender,age,medical history,etc.,and the body mass index(BMI)and waist-to-hip ratio(WHR)were calculated.The fasting elbow venous blood of the subject is collected,and the liver function,blood lipids,fasting blood glucose,fasting insulin,blood uric acid are uniformly detected by the laboratory department of our hospital,and the insulin resistance index(HOMA-IR)is calculated.The levels of serum Beclin1 and LC3 were detected by ELISA,and the levels of serum LPL and HL were detected by microplate method.Adopt SPSS 23.0 statistics software for data analysis.Results:1.Compared with the NC group,the BMI,WHR,FPG,FINS,HOMA-IR,SUA,ALT,ALP,γ-GT,CHE,TC,TG,LDL-C levels in the MAFLD group were significantly increased(P<0.05),and the levels of HDL-C,Beclin1,LC3,LPL and HL were significantly reduced(P<0.05).After testing,there was no significant statistical significance in terms of AST level,sex and age in the MAFLD group and the NC group(P>0.05).2.Compared with the NC group,BMI,WHR,FPG,FINS,HOMA-IR,SUA,LDL-C,HDL-C were helpful in judging the severity of fatty liver disease,of which the BMI in the moderate group was significantly higher than that in the mild group,and WHR increased with the severity of fatty liver(P<0.05).Beclin1 in the mild and moderate group,LC3 in the mild and severe group,LPL and HL in the mild,moderate and severe group were significantly reduced,among them,the LPL and HL of the moderate group were significantly lower than those of the mild group(P<0.05),while there was no significant statistical difference between the beclin1,LC3,LPL and HL groups of mild,moderate and severe group(P>0.05).After testing,there was no significant statistical significance in terms of sex and age in the MAFLD group and the NC group of different degrees(P>0.05).3.The level of serum Beclin1 is negatively correlated with WHR,FINS,HOMA-IR,ALT,CHE,TG;the level of serum LPL is negatively correlated with BMI,WHR,FINS,HOMA-IR,CHE and positively correlated with HDL-C;the level of serum HL is negatively correlated with BMI,WHR,FINS,HOMA-IR,CHE(P<0.05);the level of serum LC3 level is not significantly correlated with various indicators(P>0.05).4.Multivariate logistic regression analysis showed that the model containing 7variables was significant.For every 1umol/L increase in SUA,the risk of MAFLD occurred increased by 0.9%.For every 1 IU/L increase in ALP,the risk of MAFLD occurred increased by 7.9%.For every 1 mmol/L increase in TC,The risk of MAFLD occurred increased by 1.638 times.For every 1 increase in HOMA-IR,the risk of MAFLD occurred increased by 10.455 times.For every 1 kg/m~2increase in BMI,the risk of MAFLD occurred increased by 99.7%.For every 1 pg/m L increase of LC3,the risk of MAFLD occurred decreased by 0.1%.For every 1 IU/m L increase in HL,the risk of MAFLD occurred decreased by 96.9%.5.According to ROC curve analysis,Beclin1,LC3,LPL,and HL alone can effectively diagnose MAFLD.According to the Youden index,the best diagnostic performance is HL,followed by LPL,Beclin1,and LC3.Beclin1 cutoff value for MAFLD is≤8.545ng/m L(Se=0.853,Sp=0.500);LC3 cutoff value is≤5048.38pg/m L(Se=0.750,Sp=0.530);LPL cutoff value is≤0.8579 IU/m L(Se=0.706,Sp=0.733);HL cutoff value is≤0.6192 IU/m L(Se=0.765,Sp=0.727).Combined detection of Beclin1,LC3,LPL and HL,can improve the sensitivity and specificity of the diagnosis.Compared with the tests of Beclin1,LC3,LPL,and HL alone,the differences were statistically significant(P=0.001,P<0.001,P=0.018,P=0.004).Conclusion:The serum levels of Beclin1,LC3,LPL and HL in patients with MAFLD decreased significantly,and the detection of all four indicators alone can be a good diagnosis of MAFLD.Combined detection of serum levels of Beclin1,LC3,LPL and HL is more effective in the diagnosis of MAFLD patients.Combined detection of Beclin1,LC3,LPL,and HL levels is expected to become a serological marker for the noninvasive diagnosis of MAFLD,which can provide a new direction for the therapeutic targets of MAFLD.
