Objective:To evaluate the efficacy of laparoscopic sleeve gastrectomy(LSG)in the treatment of obesity complicated with hypertension.Methods:Clinical data were collected from 58 patients which with obesity and hypertension were admitted to the Third Hospital of Shanxi Medical University from June 2018 to May 2020.The method was a retrospective cohort study,and all patients underwent LSG.In this study,33 males and 25 females were aged from 18 to 64,with an average age of(37 ± 10).Outcome measures:(1)postoperative complications and remission;(2)Changes in follow-up and related indicators;(3)Relationship between various factors and remission of hypertension.The outpatient department was followed up by specific examination,telephone follow-up,wechat questionnaire filling procedure and other methods for understanding the complications,the weight loss effect and the degree of hypertension relief,respectively in 1 week and 1,3,6,12 months after the operation.Results:(1)Postoperative complications and remission: All the 58 patients underwent LSG,4 patients had acid reflux and heartburn,3 patients had significant hair loss,and 1 patient had menstrual disorder.All patients were relieved after conservative treatment;(2)Follow-up and changes of related indexes: All the 58 patients were followed up.BMI,systolic blood pressure and diastolic blood pressure have statistically significant at 1week and 1,3,6,12 months after surgery were compared with those before surgery(P<0.05).The remission rate of hypertension was 74.14% at 12 months after surgery.The difference between patients with preoperative and postoperative risk factors for cardiovascular 10 years were statistically significant(P<0.05);(3)Relationship between various factors and remission of hypertension: Patients were divided into hypertensive remission group and hypertensive non-remission group according to blood pressure remission.We compared the factors between the two groups,and found that hypertension grade,number of preoperative antihypertensive drugs taken,preoperative age,smoking status,duration of hypertension were statistically significant difference(P<0.05).Logistic regression analysis of the above significant factors showed that the number of drugs taken before surgery and smoking history before surgery were important factors affecting postoperative hypertension remission,with statistically significant difference(P<0.05).To compare the effects of 5 types of antihypertensive drugs on hypertension remission,patients taking drugs were divided into remission group and non-remission group,and it was found that the difference of β-blocker between the two groups was statistically significant(P<0.05).Conclusion:LSG treatment of obesity combined with hypertension can effectively relieve hypertension in a short term,reduce the risk and grade of cardiovascular disease in 10 years,and is expected to become a new method for the treatment of morbid obesity combined with hypertension.
