Objective:1.To compare the clinical efficacy and application value of interventional embolization and traditional laparotomy in the treatment of multiple trauma combined with blunt abdominal solid trauma.2.Epidemiological analysis of patients with multiple trauma combined with blunt abdominal solid trauma admitted to the First Hospital of Shanxi Medical University from August 2016 to July 2021.Methods:A retrospective case-control study was used to collect the medical records of inpatients with multiple trauma and blunt abdominal trauma admitted to the First Hospital of Shanxi Medical University from August 2016 to July 2021.In the medical record index system of our hospital,we retrieve the discharge diagnoses of ‘multiple trauma’ and ‘spleen injury’ or ‘liver injury’ or ‘kidney injury’,and collected data on the cause and time of the trauma.Patients who met the inclusion criteria and underwent interventional embolization with multiple trauma and blunt abdominal solid injury were enrolled as the intervention group.And according to the same main organ injury,similar ASST grades(I,II,III are marked as low-grade,IV and above are marked as high-grade injury),the same trauma mechanism,similar age,gender,similar vital signs at admission,and the baseline data with similar ISS score and GCS score are matched with patients undergoing laparotomy surgery,were enrolled as the operation group.Relevant clinical data were collected and analyzed.The operation time,blood transfusion volume,fasting time,postoperative complications,need for secondary surgery,death,ICU hospitalization days,total hospitalization days,and expenses were observed and compared between the two groups.Results:In terms of epidemiology,the traffic accidents(n=172)in 5 years,accounted for41.75% of all multiple injuries combined with blunt trauma to abdominal solid organs.The fall injuries(n=141)in 5 years,accounted for 34.22%.The violent assault injuries(n=44)within 5 years,accounted for 10.68%.The sports injuries(n=33)within 5 years,accounted for 7.77%.The other injuries(including mechanical injuries and heavy object impact injuries,etc.)(n=44)in 5 years,accounted for 5.58%.In terms of the incidence of trauma,the statistical data in the fourth year dropped significantly(n=59),accounted for14.32% of the total number of cases.In the comparison of the incidence rates of different trauma causes in each year,there was no significant difference(P>0.05).There was no significant difference between the two groups in age,gender,trauma cause,organ injury,ISS score,ASST classification,GCS score,systolic blood pressure,diastolic blood pressure,heart rate,etc.(all P>0.05).The observation indexes were comparable.The operation time,blood transfusion volume and fasting time in the intervention group were all lower than those in the operation group,and the differences were statistically significant(P<0.05).The ICU time,hospitalization time and cost of the intervention group were lower than those of the operation group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the intervention group was lower than that in the operation group,and the difference was statistically significant(P<0.05).Conclusion:(1)Epidemiological surveys showed that transportation accidents and fall injuries accounted for the vast majority of this study.Therefore,avoiding traffic accidents and fall injuries are of great significance in reducing the incidence of multiple trauma combined with blunt abdominal solid organs injury.(2)Interventional embolization technique is an effective hemostasis measure.It has a good therapeutic effect and a high success rate in the treatment of multiple trauma combined with blunt abdominal solid organ trauma.Compared with traditional laparotomy,it is not only effective,it can improve the patient’s condition,and has the advantages of short intraoperative time,short hospital stay and low incidence of postoperative complications.Interventional embolization is the first choice for hemodynamically stable patients with multiple trauma combined with blunt abdominal solid organ trauma.
