| Objective:Anxiety and depression are more common in Patients with Graves’ disease than in general patients.In this study,we use a self-assessment method to explore the proportion of anxiety and depression in patients with initial and recurrent Graves’ disease.At the same time to clear the risk factors of anxiety and depression in patients with recurrence,and to provide meaningful and more reference for the formulation of psychological nursing intervention programs for patients with Graves’ disease recurrence.Methods:From April 2021 to February 2022,95 Graves’ disease patients with initial onset and65 Graves’ disease patients with recurrence were enrolled in the First Hospital of Shanxi Medical University.In addition,80 patients with normal thyroid function and no abnormal thyroid color ultrasonic examination results were selected as the control group from outpatient physical examination patients of our hospital.The levels of FT3,FT4,TSH,TGAb and TPOAb in each group were statistically compared.Anxiety and depression was assessed by self-rating Anxiety Scale and self-rating Depression Scale.Age,education level,onset age,duration of disease,family history of hyperthyroidism and other general information of recurrent Graves’ disease patients were recorded by questionnaire.To analyze the risk factors related to anxiety and depression in patients with Graves disease recurrence.Results:1.A total of 240 subjects were included in this study,including 80 healthy controls,95 patients with initial Graves’ disease and 65 patients with recurrent Graves’ disease.There were no statistically significant differences in gender,age and years of education between the groups(P>0.05),and the data were matched.2.Compared with healthy controls,the proportion of anxiety and depression in Graves patients(including patients with initial onset and recurrence)was significantly higher(P<0.05),and SAS and SDS scores were higher than those in healthy controls(P<0.05),the difference was statistically significant.3.Compared with patients with initial Graves disease,the proportion of anxiety and depression in patients with recurrent Graves disease was significantly higher,and SAS and SDS scores were higher than those in patients with initial Graves disease(P<0.05),the difference was statistically significant.4.Compared with healthy controls,FT3 and FT4 were increased,TSH was decreased,TPOAB and TGAB were increased in patients with initial Graves disease(P<0.05),the difference was statistically significant.Compared with healthy controls,FT3 and FT4 were increased,TSH was decreased,TPOAB and TGAB were increased in patients with Graves’ disease recurrence(P<0.05),the difference was statistically significant.5.The influencing factors of anxiety and depression were analyzed by Logistic regression.The anxiety of recurrent patients was related to age of onset,course of disease,personality,occupational status,family history of hyperthyroidism,goitre,combination of medication,compliance,coping style and other factors.Depression was related to age of onset,course of disease,personality,occupational status,family history of hyperthyroidism,goiter,combination of medication,compliance,coping style and so on.6.Multivariate Logistic regression analysis: young onset age,long course of disease,introversion and goiter were risk factors for anxiety in patients with Graves’ disease recurrence(OR=0.707,OR=1.375,OR=10.353,OR=7.185).Young onset age,long course of disease,introversion and engaging in mental work were risk factors for depression in patients with Graves’ disease recurrence(OR=0.848,OR=1.408,OR= 4.678,OR=5.052).Conclusion:Patients with recurrent Graves’ disease are more likely to have anxiety and depression than healthy people and patients with initial disease.Age of onset,course of disease,personality and goiter were risk factors for anxiety in recurrent Graves patients.Age of onset,course of disease,personality and mental work were risk factors for depression in recurrent patients. |
