| Objective:At present,there are few reports on the risk factors and mechanism of hydrocephalus after hemorrhage.This study aims to explore the changes of total protein of cerebrospinal fluid in patients with hydrocephalus after hemorrhage and its predictive value for hydrocephalus after intracranial hemorrhage.Methods:The clinical data of 156 patients with intracranial hemorrhage in our hospital were retrospectively analyzed.According to the prognosis,they were divided into posthemorrhage hydrocephalus group(PHH group)and non-posthemorrhage hydrocephalus group(NPHH group).The clinical data were analyzed by univariate analysis,and multivariate analysis was performed by binary logistic regression after the interference factors were excluded.The CSF total protein(CSF-TP)of the patients in the last cerebrospinal fluid test was linearly analyzed with time,and the incidence of PHH after the CSF-TP was grouped with time was analyzed.Results:Among 156 patients with intracranial hemorrhage,85 patients(54.5%)eventually developed posthemorrhage hydrocephalus,of which 34 patients(21.8%)were treated by lateral ventriculoperitoneal shunt surgery.Risk factors included hypertension(P = 0.017),duration of fever(days with body temperature ≥ 38.5℃,P =0.040),GCS(P < 0.001)and time(onset time of CSF-TP,P < 0.001)was correlated with PHH occurrence.Binary Logistic regression analysis showed that GCS< 8 [OR= 2.943(1.421~6.097),P = 0.004] and ≥ CSF-TP × time [OR = 2.317(1.108~4.849),P= 0.026] were independent risk factors for PHH.All CSF-TP values were divided into groups every 2 days,and the mean CSF-TP values of each group were taken.Linear analysis showed that CSF TP was negatively correlated with time(P < 0.01).In the same time of onset,the level of CSF-TP in PHH group was significantly higher than that in NPHH group.Overall,the increase of CSF-TP in PHH group lasted longer.Conclusion:The CSF-TP level in PHH group and NPHH group was statistically different,GCS < 8 and CSF-TP× time(≥ 9,600)were independent risk factors for PHH.The distribution of CSF-TP in PHH group was significantly higher than that in NPHH group.. |
