Clinical Study On The Effect Of Bugu Yishen Decoction On Symptom Improvement And Bone Metabolism Indexes Of Elderly Patients With OVCF After Vertebroplasty

Objective: With the aging of the population,the incidence of osteoporotic fractures in the elderly is increasing,among which the thoracolumbar compression fractures are the most common.Osteoporotic Vertebral Compression Fracture(OVCF)is a pathological bone disease based on osteoporosis.Vertebroplasty is currently the most common surgical treatment for OVCF in the elderly.It can quickly relieve the pain caused by fracture and improve the function limitation of the lower back.The injection of bone cement can play a role in rapid reduction,support and pain relief of the affected vertebra.However,postoperative osteoporosis related symptoms were not effectively treated.We have accumulated a large amount of experience in the treatment of elderly OVCF with Traditional Chinese medicine.In this study,oral Bugu Yishen Decoction was used to investigate the improvement of symptoms after vertebroplasty in elderly Patients with OVCF and the effect on bone metabolism indexes.Methods:Aged OVCF patients who received treatment and surgery in the Department of Orthopaedics of the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine from November 2020 to November2021 were selected.They ranged in age from 60 to 90,and there were 90 of them.Their imaging findings were fresh osteoporotic vertebral compression fractures of a single thoracolumbar body.The patients were randomly divided into three groups: group A,group B and group C,all of whom underwent vertebroplasty.After surgery,group A was orally treated with Bugu Yishen Decoction,group B with calcium carbonate D3 tablets,and group C with Bugu Yishen Decoction and calcium carbonate D3 tablets.The improvement of symptoms and bone metabolism indexes were compared before and after treatment.Results: The following results are obtained through this study.1.There were statistically significant differences between groups A,B and C after treatment and before treatment(P < 0.05),indicating that treatment was effective.2.Visual VAS pain score in all three groups decreased after treatment,and there were statistical differences before treatment,indicating that the treatment was effective;After treatment,ODI dysfunction index in all three groups decreased,with statistical difference(P < 0.05),indicating that treatment was effective.3.The β-CTX and PICP indexes of the three groups after treatment were statistically different from those before treatment,indicating that the treatment was effective,and the efficacy of group A and B was similar,while the efficacy of group C was significantly better than that of group A and B.4.The 25-hydroxyvitamin D levels in the three groups after treatment were statistically different from those before treatment,indicating that the treatment was effective,and the efficacy of group B and C was similar,while the efficacy of group A was inferior to that of group B and C.Conclusion: Taking Bugu Yishen Decoction and taking calcium carbonate D3 tablets are effective for the improvement of osteoporosis symptoms and bone metabolism indicators,the combined therapy of the two is better.However,this study found that in the treatment of 25-hydroxyvitamin D,the treatment effect of group B and C taking calcium carbonate D3 tablets was better than that of group A taking oral Chinese medicine alone.
