Abstract/Summary:
1.Objective This study is to observe the relationship between post-meal hypotension(PPH)and heart rate variability(HRV)and blood clots in elderly hospitalized patients, and to provide reference for clinical diagnosis of post-meal hypotension and treatment.2.MethodsCollect 95 cases of patients with low blood pressure after meals in the Department of Geriatric Medicine of Shandong University of Traditional Chinese Medicine from March 2020 to December 2021,and record the patient’s general clinical data and quantitative score of blood clots. After 24 h dynamic blood pressure monitoring, according to the post-meal hypotension diagnostic criteria,it is divided into after-dinner low blood pressure group(PPH group)and non-meal low blood pressure group(non-PPH group),and through 24 h dynamic electrocardiogram monitoring compared the difference between the two groups of patients heart rate variability indicators,as well as after-dinner low blood pressure, heart rate variability and severity of the blood clot correlation. The data obtained from this study will be analyzed statistically by SPSS 25.0 statistical software, the measurement data will be represented by (?)± S,and the average between the two groups will be compared to the t-test for analysis;P<0.05 is statistically different. The relevant factors are included in the logistic regression model analysis. The test level of P is 0.05,and the difference of P<0.05 is statistically significant.Data that did not conform to the normal distribution were tested with nonparametrics, and The Monitorman-related analysis was used to analyze the correlation between the factors of whether or not hypotension after meals and the quantitative score of haemorrhoids were used as factors.3.ResultsThe total detection rate of low blood pressure after meals was 67.4% in elderly hospitalized patients,of which the highest detection rate of breakfast PPH was 84.4%.Comparison of PPH group and non-PPH group heart rate variability index: The test results showed that the PPH group SDNN,RMSSD,PNN50 index was significantly lower than non-PPH group, and the PPH < 0.01,indicating that the difference between the two groups of patients was statistically significant.Based on whether PPH occurred as a factor variable, the patient’s age, heart rate variability index, hypertension, diabetes, paraplegia as the independent variable, the results show that the heart rate variability index SDNN(r-0.842,P-0.025),PNN50(r-0.440,P-0.015),hypertension(r-69.266,P-0.041),paraplegia(r-46.181,P-0.030)was significantly correlated with PPH occurrence, with PNN50 being most relevant to Whether PPH occurred or not,and age, diabetes and PPH being not significantly correlated. As a dependent variable, the blood clot quantitative score was analyzed with the Pearson correlation of heart rate variability index, age, hypertension, diabetes, paraplegia, and whether PPH occurred: the blood clot quantitative score was negatively correlated with SDNN(P-0.01).The quantitative score of haemorrhoids was positively correlated with diabetes(P-0.008),paraplegia(P-0.001),and Whether PPH(P-0.001)occurred.Significant correlation with whether PPH occurs.4.ConclusionThe decrease of autonomic neurobalance in elderly hospitalized patients is closely related to the occurrence of PPH,and PPH patients may have a disorder of the autonomic nervous system of the heart. At the same time, the higher the blood score, the heavier the blood evidence, the greater the risk of low blood pressure after meals, the lower the patient’s heart rate variability index. It is shown that haemorrhoids are an important risk factor for the occurrence of hypotension after meals.
