Objective: Use superb microvascular imaging(SMI)to analyze the microvascular distribution of breast nodules on BI-RADS category 4 detected by automated breast volume scanner(ABVS),and to explore the value of ABVS combined with SMI in the diagnosis of breast nodules on BI-RADS category 4.Methods: Selected 82 female patients who had ABVS from September 2020 to September 2021,a total of 87 breast nodules on BI-RADS category 4 were detected and adjusted for their BI-RADS classification combined with SMI.Results:1)Among the 87 breast nodules on BI-RADS category 4 detected by ABVS,55(63.2%)were benign and 32(36.8%)were malignant,and the diagnostic accuracy was 83.9% in 4a(47/56);58.8% in 4b(10/ 17);92.9% in 4c(13/ 14),the difference was statistically significant(P < 0.05).2)In SMI mode,the blood flow distribution of benign nodules is mainly displayed avascular(18.2%),line-like(36.4%),and branch-like(30.9%)patterns,and malignant nodules tended to display root hair-like(31.3%)and crab claw-like(37.5%)patterns,the difference between the microblood flow distribution of breast benign and malignant nodules is statistically significant(P < 0.05).3)In SMI mode,the RI of blood flow in malignant nodules(0.75 ± 0.05)was significantly higher than that in benign nodules(0.62 ± 0.06)(P < 0.05).4)The area under ROC curve of ABVS and ABVS combined with SMI were 0.787 and 0.932,the sensitivity was 71.9% and 93.8%,the accuracy was 80.5% and9 3.1%,the negative predictive value was 83.9% and 96.2%,the difference was statistically significant(P < 0.05);the specificity was 85.5% and 92.7%,and the positive predictive value was 74.2% and 88.2%,there was no significant difference(P > 0.05).Conclusion:1)In SMI mode,the distribution of blood flow of root hair-like and crab claw-like patterns tended to malignant nodules,while that of avascular,line-like type and branch-like patterns tended to benign nodules.2)In SMI mode,blood flow RI in malignant breast nodules were higher than blood flow RI in benign nodules.3)ABVS has high sensitivity,specificity and accuracy in the diagnosis of breast nodules on BI-RADS category 4.The combination of ABVS and SMI can significantly improve the diagnostic efficiency. |
