| Objective: When the treatment of psoriasis is now dominated by " curing from blood theory ",but a complete theoretical system has not been establied.Professor Qin Wan Zhang proposed the concept of " new blood syndromes ".In this study,we conduct a real-world study on the basis of " new blood syndromes " in psoriasis,analyze these indexes by PASI,BSA,DLQI,vas and TCM syndrome score,and understand the efficacy and safety of treating psoriasis on the basis of this theory,and form real-world evidence to solve the lack of real-world data on " new blood syndromes " in psoriasis Question of high-level clinical evidence.Methods: Using an observational,prospective cohort study,psoriasis patients in dermatology department of traditional Chinese Medicine Hospital Affiliated to Southwest Medical University,was divided into blood fever syndrome group,blood stasis syndrome group,non blood fever non blood stasis syndrome group according to the diagnostic criteria of the traditional Chinese medicine(TCM)of psoriasis,and 25 patients who met the criteria were sequentially enrolled in each group to implement life guidance to the patients in each group and issue urea cream as the basic moisturizing care,After being orally administrated with symptomatic drugs for 8 weeks and followed up for another 8 weeks,the corresponding indicators(including PASI,BSA、DLQI,VAS scores,adverse events,recurrence,etc.)at week 0,4,8 and 16 were recorded in patients;Another 25 non psoriatic patients were included as a healthy control group without intervention treatment,and the general condition of patients and the physical type of TCM were recorded.Finally,the data were statistically analyzed using SPSS 23.0 software to understand the efficacy and safety of "" new blood syndromes ""-based TCM treatment options for psoriasis and form real-world evidence,thus completing the theory construction of "" new blood syndromes "" in psoriasis.Result:A total of 100 cases were included in the study,and shedding,culling,discontinuation,and other cases were excluded,resulting in a total of96 cases analyzed.1.Using traditional medicine to treat psoriasis is safe and effective.(1)The general data of patients were not significantly different between groups(P>0.05),and the analysis showed that the proportion of patients with blood fever syndrome whose disease duration was less than 1month was higher than the remaining two groups,while the proportion whose disease duration was greater than 12 months was lower and the chance of infection induced blood fever syndrome was greater than the other two groups.(2)The affected quality of life was higher in the group with blood fever syndrome than in the group with blood stasis syndrome before handle,whereas it was conspicuous in the group with blood stasis syndrome on TCM for the other two(P < 0.05).(3)There was a conspicuous lessen in all three groups in balance with the PASI before treatment and at week 8,while there was no conspicuous distinguish between the groups in balance of the PASI before treatment and at week 4 and between the PASI groups at each follow up time(P > 0.05);There was no significant difference in pre-and post BSA or intergroup contrast at each time point of treatment(P > 0.05);DLQI,vas and TCM syndrome scores of patients with blood heat syndrome and non blood heat non blood stasis syndrome at the 4th week of treatment were conspicuous lessen in balance with those untreated,and the above indexes were evidently transform in patients from the 8th week of treatment compared with those before,and the VAS of patients with blood stasis syndrome were conspicuous increased compared with those at the 8th week after treatment(P < 0.05),with no significant difference between groups in the various time points of vas(P >0.05).(4)The overall response rate was 46.5% at the 4th week of treatment,78.9% at the 8th week,and 32.4% at the 8th week after treatment,the percentage of PASI decline in each group at the 8th week of treatment was conspicuous higher than that at the 4th week(P < 0.05),and the differences among the three groups were not statistically significant(P > 0.5).Conclusion:1.Using traditional medicine to treat psoriasis is safe and effective.2.Psoriasis treatment based on the " new blood syndromes " of psoriasis can safely and effectively improve the severity of skin lesions,itching symptoms,traditional Chinese medicine(TCM)syndrome,and reduce the impact of psoriasis on quality of life.3.Psoriasis " new blood syndromes " in clinical application is effective in patients with different syndromes,and the efficacy of each syndrome is comparable.4.The suggested course of treatment of psoriasis based on the regimen " new blood syndromes " of psoriasis is more than 4 weeks,and the course is positively correlated with the efficacy. |
