A Research On The Real World Clinical Diagnosis And Treatment Data Of Traditional Chinese Medicine For Chronic Bronchitis

Objective:To explore the feasibility of Real World Study methods in the summary and application of famous and old traditional Chinese medicine experience;Data mining and sorting out the prescription rules and drug characteristics of Shen Chunti's clinical experience in the treatment of chronic bronchitis by using the Traditional Chinese Medicine Inheritance Support System;To identify the new mode of diagnosis and treatment of teacher's disease syndrome type in the clinical application of traditional Chinese medicine through the analysis of Shen's clinical experienceMethod:I.Literature research:through the reading and analysis of traditional and contemporary documents,the definition and diagnostic criteria of chronic bronchitis and its relationship with emphysema and chronic obstructive pulmonary disease are clarified;the research progress of traditional Chinese medicine on the name,etiology,pathogenesis,syndrome type and clinical treatment of chronic bronchitis is summarized to provide the basis for clinical real world research2.Design method:using the clinical epidemiological research method,in the real and impartial group of traditional Chinese medicine in the diagnosis and treatment of chronic bronchitis,to study the actual application of traditional Chinese medicine in the treatment of chronic bronchitis,using the prospective time sequence and self-control design method,to observe the differences of indicators before and after the treatment of chronic bronchitis,and to explore the effect of clinical intervention measures for this disease3.Data mining:the multi factor and complex correlation that traditional Chinese medicine attaches importance to is the cause of the balance destruction caused by the change of the relationship between the body and the environment,which is in line with the real world research model,and the computer data mining technology makes it possible to interpret this complex relationship.In this paper,302 cases of chronic bronchitis treated by Professor Shen Chunti were collected on the spot,and data mining research such as association rules,entropy cluster analysis was carried out.4.Observation of clinical effect:SPSS 25.0 software was used to analyze the dynamic effect of 106 cases of chronic bronchitis treated by teacher Shen Chunti before and after clinical treatment.Result:1.Ancient and contemporary literature research,clear the concept of chronic bronchitis related to Chinese and Western medicine:there was no name of chronic bronchitis in ancient Chinese medicine,mainly discussed from three aspects of cough,phlegm and asthma,and Western medicine defined the definition and clinical diagnosis standard of chronic bronchitis.Chronic bronchitis can be complicated with emphysema,or progress to chronic obstructive pulmonary disease,but the three diseases exist independently,cross each other and can not completely cover either side.In Chinese medicine,there is no unified standard and treatment plan for the etiology,pathogenesis and syndrome differentiation of chronic bronchitis2.Data mining analysis of Professor Shen Chunti's clinical experience in the treatment of chronic bronchitis:Professor Shen's clinical treatment of chronic bronchitis is based on the core pathogenesis of lung deficiency and phlegm depression,and the most common cause of acute exacerbation of chronic bronchitis is the wind pathogen induced phlegm stagnation.Professor Shen's clinical medication is mainly based on mild cold,combined cold and warm,united ascend and descent,and considered dredge and tonic drugs.Based on ephedra,bitter almond,scutellaria,fritillaria thunbergii,qianhu,aster,perillaseed and raphanus seed,they can stop cough,eliminate phlegm and asthma.Then,according to different syndrome elements and clinical manifestations,we can carry out syndrome differentiation and treatment,dynamic use principle drug,or dispelling wind and eliminating lung,or clearing away lung and resolving phlegm,or warming the lung to reduce watery phlegm,or manage lung and remove blood stasis,or tonify the kidney to improve inspiration.3.Comparison of clinical efficacy before and after treatment:the total effective rate was 48.11%and the symptom scores of cough,expectoration and wheezing decreased in 106 patients who were finally included in the study after 7 days of treatment;the total effective rate was 97.17%and the symptom scores of cough,phlegm and wheezing decreased significantly after 14 days of treatment;the trend test before and after treatment was P<0.001,suggesting that with the increase of treatment visits,the effective rate of Professor Shen in the treatment of chronic bronchitis showed a significant upward trend.Conclution:Real world research is an evaluation study of clinical effect and patient feedback.It can select treatment measures according to the actual condition and willingness of patients,and evaluate the comprehensive effect of the overall treatment.Therefore,real world research is an effective and feasible research method to promote the high-quality inheritance and development of famous and old TCM experience.Data mining analysis and dynamic effect observation are two common treatment methods of TCM effectiveness evaluation method.Through data mining,this study clarifies the medication rules and characteristics of Professor Shen in the diagnosis and treatment mode of chronic bronchitis.Through self comparison,it is confirmed that Professor Shen 's clinical treatment of chronic bronchitis is more effective,and the diagnosis and treatment mode of disease and syndrome has clinical value.
