Correlation Of Plasma Gal-3、GDF-15、ANP、sST-2 Concentrations And Tei Index In Patients Dilated Cardiomyopathy Combined With HFrEF

Objective:To investigate the correlation between plasma Gal-3,GDF-15,ANP and sST-2concentrations and Tei index in patients with dilated cardiomyopathy combined with HFr EF,and to provide a theoretical basis for the diagnosis and severity assessment of patients with dilated cardiomyopathy combined with HFr EF.Methods:Fifty-five patients with dilated cardiomyopathy combined with HFr EF diagnosed at the Department of Cardiology of the Affiliated Hospital of Inner Mongolia National University from January 31,2021 to January 31,2022 were randomly selected as the experimental group and divided into cardiac function class II(n=18),class III(n=18),and class IV(n=19)groups according to the New York Heart Function Classification(NYHA).Meanwhile,47 patients diagnosed with HFp EF in the same period in our hospital were selected as the control group.General clinical data of the study subjects were collected,while plasma Gal-3,GDF-15,ANP,and sST-2 concentrations were measured by ELISA in both groups;left atrial internal diameter(LAD),left ventricular end-diastolic internal diameter(LVEDd),left ventricular end-systolic internal diameter(LVESd),interventricular septal thickness(IVSd),left ventricular ejection fraction(LVEF)and calculation of the Tei index.The biochemical and ultrasound indices of the two groups were compared,and the changes of Gal-3,GDF-15,ANP and sST-2 in different cardiac function classifications in patients with dilated cardiomyopathy combined with HFr EF,and the correlation between Gal-3,GDF-15,ANP and sST-2 and echocardiographic indices were analyzed.Statistical analysis was performed using SPSS26.0 statistical software,and differences were considered statistically significant at P<0.05.Results:1.Comparison of baseline data between control and experimental group patients: age,gender,hemoglobin,blood glucose,total cholesterol,LDL,transaminases,blood creatinine and urea nitrogen were not statistically different(all P > 0.05).Heart rate and blood creatinine were greater in the experimental group than in the control group,with no statistical difference(all P > 0.05).2.Comparison of biomarkers between the control group and the experimental group:compared with the control group,plasma Gal-3,GDF-15,ANP and sST-2 levels were significantly higher in the experimental group(P<0.05);Gal-3,GDF-15 and ANP levels gradually increased and differed significantly among the groups of cardiac function class II-IV(all P<0.05);sST-2 levels were significantly higher among the subgroups of cardiac function class II-IV(all P<0.05).There was no significant difference in sST-2 levels among subgroups of cardiac function class II-IV(P>0.05).3.The Tei index,LAD,LVEDd and LVESd values in the experimental group were higher than those in the control group,and LVEF values were lower than those in the control group.In the NYHA cardiac function class II-IV group,LVEF gradually decreased,while Tei index,LAD,LVEDd and LVESd gradually increased and the difference was statistically significant(P<0.05).4.The correlation analysis of plasma Gal-3,GDF-15,ANP and sST-2 with NT-Pro BNP,LAD,LVEDd,LVEF,Tei index and NYHA in the experimental group showed that Gal-3,GDF-15 and ANP were positively correlated with NT-Pro BNP(r values = 0.48,0.37 and0.39,respectively,P < 0.05);Gal-3,GDF-15,and ANP were significantly and positively correlated with LAD(r values = 0.92,0.45,and 0.62,respectively,P < 0.001);Gal-3 and ANP were positively correlated with LVEDd(r values = 0.51 and 0.30,respectively,P <0.05);Gal-3,GDF-15,and ANP were negatively correlated with LVEF values(r=-0.63,-0.51,-0.42,P<0.05);Gal-3,GDF-15,ANP and Tei index were positively correlated(r=0.60,0.44,0.39,P <0.05);Gal-3,GDF-15,ANP and NYHA cardiac function class were significantly positively correlated(r values=0.94,0.58,and 0.54,respectively,all P<0.001);sST-2 was not significantly correlated with NT-Pro BNP,LAD,LVEDd,LVEF,Tei index,and NYHA(all P >0.05);Tei index was positively correlated with NYHA(r=0.57,P<0.001);Tei index was negatively correlated with LVEF values were negatively correlated(r=-0.39,P <0.05).Research Conclusions:Plasma Gal-3,GDF-15 and ANP levels were significantly and positively correlated with Tei index and NYHA classification,which are good biomarkers of heart failure.
