OBJECTIVE:To evaluate the acute hemodynamic effects of urapidil in dilated cardiomyopathy patients with heart failure.METHODS:Twelve patients with heart failure due to dilated cardiomyopathy received 25mg urapidil by intravenous injection for 20mm. Before and 10, 30, 60 ,90min after management, Right atrial pressure (RAP), mean pulmonary artery pressure (MPAP),mean pulmonary capillary wedge pressure (PWP) were measured by Swan-Ganz catheter. Cardiac output (CO) was measured with the thermodilution technique. Systemic artery pressure, including systolic blood pressure (SBP), diastolic blood pressure (DBP) and mean arterial pressure(MAP), was measured by non-invasive cuff. Heart rate (HR) was recorded through cardiac monitor. Cardiac index (CI), systemic vascular resistance index (SVRI), pulmonary vascular resistance index (PVRI), left ventricular stroke work index (LVS WI) were calculated.3RESULTS:Patients had normal SBP, DBP, MAP, but HR, RAP, MPAP, PWP, SVRI, PVRI were greater and CO, CI, LVSWI were lower than normal before treatment. Ten minutes after administration, DBP, MAP, MPAP, PWP, SVRI, PVRI decreased (P<0.05) ,CO, CI, LVSWI increased (P<0.01) ; A maximum decrease in DBP, MAP, MPAP, PWP, SVRI, PVRI (P<0.01) and a maximum increase in CO, CI, LVSWI (P<0.01) was observed at 30mm after treatment; At 60mm the level of DBP, MAP, MPAP, PWP, SVRI, PVRI began to increase, and CO,CI, LVSWI began to reduce; 90mm after the treatment, DBP, MAP increased to the level before treatment, MPAP, PWP, SVRI, PVRI were lower than before (P < 0.05) , CO, Cl, LVSWI decreased to the level before treatment (P<0.05) .There were no significant alteration in HR, SBP, RAP. No side effects were observed in the study.CONCLUSIONS:Urapidil can improve the acute hemodynamic parameters in dilated cardiomyopathy patients with heart failure, be well tolerated and could be selected in acute phrase.
