Objective:To compare the high flow nasal cannula(HFNC)with conventional oxygen therapy(COT)in patients with severe bronchial asthma complicated with respiratory failure,and to explore a new innovative therapy.Methods:From November 2019 to November 2021,patients with severe bronchial asthma complicated with respiratory failure who met the admission criteria were enrolled in the Affiliated Hospital of Inner Mongolia University of Nationalities.60 patients were randomly divided into treatment group(30 cases)and control group(30 cases).All patients received standard treatment including short acting β-2agonist,inhaled corticosteroids,and antibiotics(if required)and so on.In the treatment group,HFNC was used,while in the control group,COT was administered via nasal cannula.The degree of dyspnea,heart rate(HR),respiratory rate(RR),mean arterial pressure(MAP),oxygen saturation,arterial blood gas analysis was measured before and after oxygen inhalation therapy at 6 hours,12 hours,24 hours,and 48 hours.But comfort scale in patients were analyzed after the treatment i.e.,48 hours.SPSS 20.0 was used to entry data and analyzed using independent t-test.Results:In patients with bronchial asthma with respiratory failure for both groups,HR was reduced(98.47±2.63 vs 73.13±3.04,97.27±3.40 vs 76.97±2.41,p<0.05),RR was also reduced(31.13±2.40 vs 17.27±0.86,30.60±2.60 vs 20.27±1.20,p<0.05),whereas partial pressure of oxygen was effectively elevated(53.3±2.93 vs 91.47±7.74,55.17±2.96 vs 85.33±4.01,p<0.05)after the treatment.While blood pH was elevated(7.32±0.14 vs 7.39±0.32,7.34±0.27 vs 7.39±0.37,p>0.05)and partial pressure of carbon dioxide(PCO2)was also elevated(31.37±2.48 vs 36.93±1.63,30.70±2.13 vs 37.50±2.33,p>0.05).No statistically significant difference in the effect on blood pH and PCO2 were found between two groups(p>0.05).And also,dyspnea score and comfort scale of both groups were satisfactorily reduced at the end of treatment(4.00±0.45 vs 1.10±0.30,4.07±0.45 vs 1.53±0.50,p<0.05)and(9.20±0.66 vs 6.83±0.95,p<0.05).Conclusion:In the treatment of patients with severe bronchial asthma complicated with respiratory failure,both HFNC and COT were consistent in terms of overall clinical response rate,but HFNC could significantly elevate PaO2 and reduce HR and RR.And,HFNC could achieve the same result as COT in less time.Patients treated with HFNC were more comfortable than that of COT group.Therefore,HFNC is a new choice in oxygen therapy for patients with severe bronchial asthma complicated with respiratory failure. |
