Functional Gastrointestinal Disorders:psychosomatic Assessment And Factors Affecting Quality Of Life

Abstract/Summary:
Objective:To complete the systematic psychosomatic evaluation of patients with FGIDs and identify their combined psychosocial problems,and to analysis the factors affecting gastrointestinal quality of life.Methods:From February 2020 to February 2021,99 outpatients or inpatients who met the Rome Ⅳ Criteria for FGIDs were selected in the Department of Gastroenterology,the Third Affiliated Hospital,Soochow University.Before treatment,all these patients accepted psychosomatic assessment by use of questionnaires,including GAD-7,PHQ-9,PHQ-15,DCPR-R-SSI and MPQ.In addition,GIQLI was applied to quantify the gastrointestinal quality of life,and then factors can be analyzed.Results:Among the patients with FGIDs,the prevalence of traditional psychological assessment(anxiety,depression and somatization)syndromes was 59.6%(n=59),61.6%(n=61)and 75.8%(n=75)respectively.The prevalence of DCPR syndromes was 94.9%(n=94),and Type A behavior(n=66,66.7%),persistent somatization(n=63,63.6%),and allostatic load(n=39,39.4%)were the most frequent diagnostic findings.The prevalence of DCPR syndromes was 1.2 times that of traditional psychological assessment,and the number of DCPR syndromes was positively correlated with GAD-7 score,PHQ-9 score and PHQ15 score(r=0.466;r=0.505;r=0.555,P<0.05).There was a positive correlation between MPQ score and GAD-7 score,PHQ-9 score,PHQ-15 score,the number of DCPR syndromes,allostatic load,disease phobia,demoralization(r=0.683;r=0.680;r=0.545;r=0.468;r=0.368;r=0.227;r=0.364,P<0.05).Multiple linear regression analysis suggested that anxiety,depression,somatization and psychosomatic disorders were independent risk factors of gastrointestinal quality of life.Conclusion:The psychosocial comorbidities(anxiety,depression,somatization and psychosomatic disorders)of FGIDs are prevalent in the clinical practice.The DCPR assessment,related to traditional psychological assessment,is more sensitive to screening psychosocial problems.The state of anxiety and depression,somatization and psychosomatic disorders are influencing factors of gastrointestinal quality of life of patients with FGIDs,hence psychosomatic assessment can help physicians to develop individualized strategies of psychosomatic intervention more efficiently so as to improve the quality of life of FGIDs patients.
