A Network Pharmacological Study On Vestibular Neuritis And The Mechanism Of Action Of Sangju Yin With Integrated Traditional Chinese And Western Medicine

Objective:To explore the clinical therapeutic effects of mulberry drinking flavor formula combined with hormone and peripheral vestibular rehabilitation exercise for vestibular neuritis,focusing on patients with wind evil external type vestibular neuritis,so as to bring a safe and effective set of treatment protocols for.We subsequently conducted a network pharmacology study on the action mechanism of mulberry inulin,after the effective pharmaceutical ingredients,action targets,connection pathways,in order to analyze the common link between them and vestibular neuritis.Materials and methods : A total of 70 VN patients diagnosed in the otorhinolaryngology outpatient clinic of Liaoning Traditional Chinese medicine hospital from September 2019 to January 2021 were selected as the study subjects,and the above confirmed patients were randomly divided into a control group(combination of hormone oral and rehabilitation exercise)and a test group(combination of mulberry drinking tastant formula combined with hormone oral and rehabilitation exercise therapy),35 control group,35 test group,7 days as one course,after clinical continuous treatment for two courses,observe the symptoms and signs of the two groups before and after treatment,according to the examination results of double temperature test,video head pulse test(v-HIT),vertigo disability scale(DHI)and vestibular symptom index(VSI),the clinical efficacy of which was evaluated.Then,a database of mulberry drinking components was established through the traditional Chinese medicine systems pharmacology database and analysis platform(TCMSP),and potential active components of mulberry drinking were collected and screened to find effective targets of action,form a disease target database in the context of the human gene database(Genecards),link relevant gene sets Venn diagram,and rely on the database and interaction of protein interaction network(STRING)Figure software Cytoscape3.7.2,the protein interaction PPI analysis relationship network diagram and the regulatory network diagram of TCM ingredient disease were produced to analyze Go and KEGG enrichment pathways.Result:1.The above test group dropped out 2 cases during the course of treatment,and finally 68 cases after 2 weeks of clinical treatment,the indices of each subscale(DHI-P,DHI-F,DHI-E DHI total)of vertigo disability scale in both groups were reduced compared with those at admission,and the test group considered that the vertigo was accompanied by fewer effects before and after treatment(P < 0.05).Six items of symptom relief in vestibular visual analog scale were better than that in the control group(P < 0.05).The changes were detected by double temperature test before and after treatment in both groups Of CP values,with a greater difference between the test groups before and after treatment(P < 0.05).2.The gain values were recorded before and after treatment using a video head pulse test system for comparison.The test group outperformed the control group and could recover to the normal range more quickly(P < 0.05).Having collated that the mulberry drink contains a total of 202 active ingredients after screening,a total of 38 intersection target genes,including VEGFA,CASP8,ALB,ESR1,ACh E,EGFR and others,that act on VN in mulberry drinking were analyzed by Venn.Finally,GO and KEGG pathways were enriched.Conclusions:1.After clinical trials have confirmed that mulberry drinking plus flavor formula combined with hormone and peripheral vestibular rehabilitation exercise is more effective and feasible to be clinically promoted for the treatment of wind evil external type vestibular neuritis.2.Through the network pharmacology analysis of the mechanism of Sangju drink in the treatment of VN,we know that the effective compound targets in many traditional Chinese medicines inhibit the viral invasion by affecting the replication stage of herpes virus cleavage phase;At the same time,the interaction of protein between Sangju drink and VN is mainly used to regulate Caspase-3 and Bax protein to improve the blood supply of vestibular nucleus,so as to clear oxygen free radicals and increase vascular permeability;DRD2 participates in vestibular compensation,ACh E promotes the recovery and regeneration of neurons,and so on.Kaempferol and quercetin contained in many traditional Chinese medicines can inhibit the inflammatory reaction of central nervous system and improve the body's autoimmunity,so as to alleviate the symptoms of VN.Finally,through the enrichment analysis of GO and KEGG pathway,the relationship between go and vestibular neuritis was elaborated.3.In this paper,through the design of experiments to observe the clinical effect of integrated traditional Chinese and Western medicine treatment,the use of network pharmacology to analyze the relationship between drugs and diseases,to provide a sufficient basis for clinical treatment.4.Traditional Chinese medicine decoction is added to the treatment of vestibular neuritis,which is in line with the traditional Chinese medicine view of people-oriented and overall harmony.With the change of the disease,the traditional classical prescriptions are modified according to the syndrome,with high safety and easier to accept by patients.
