| Purpose:In this study,we retrospectively analyzed the factors related to the survival of patients with brain metastases from lung cancer,to explore whether the Chinese medicine and other treatment methods can effectively prolong the survival time of patients with brain metastases from lung cancer.Material and method:Patients with brain metastases from lung cancer who attended the Oncology Department of the Liaoning University of Traditional Chinese Medicine from March 1,2018 to October 31,2020 were selected for retrospective analysis.A total of 62 patients met the inclusion criteria,the patient's name,sex,age,history of smoking,survival time after brain metastasis,age of onset,KPS score,initial operation,lung radiotherapy,initial chemotherapy,initial targeted therapy,chemotherapy,radiotherapy,targeted therapy,immunotherapy and high exposure therapy of traditional Chinese medicine were analyzed. WPS software was used to record the basic data of patients and SPSS 21.0 software was used for survival analysis.Results:1.A total of 62 patients(50.0%)who met the criteria were collected,and 31 patients died by the time of investigation.There were 37 males(59.6%),25 females(40.3%).The median age was 63 years old.The MST of 62 patients with brain metastases from lung cancer was 14 months,the shortest was 2 months,the longest was 53 months,and the 1-year survival rate was 63.59%.2.There were 64 prescriptions in total,189 kinds of Chinese herbal pieces were used,the total frequency of Chinese herbal pieces was 1625 times,the average was about 22 Chinese herbal pieces per square,and 51 Chinese herbal pieces were screened out.3.Main clinical symptoms: symptoms with a total frequency greater than 10 times were analyzed,the longest clinical manifestations of lung cancer patients with brain metastasis were: fatigue in 36 cases(58.0%),cough in 30 cases(48.3%),shortness of breath in 22 cases(35.4%),dizziness in 22 cases(35.4%),drowsiness in 18 cases(29.0%),accommodation in17 cases(27.4%),limitation of limb movement in 17 cases(27.4%),headache in 16 cases(25.8%),constipation in 14 cases(22.6%),expectoration in 14 cases(22.5%),nausea in 12cases(19.3%),emaciation in 11 cases(17.7%).According to the four diagnostic data,15 cases(24.2%)were classified as liver-wind type,12 cases(19.3%)as liver-wind type,10 cases(16.1%)as liver-collateral type and 25 cases(40.3%)as zheng-xu-xie-lian type.Every syndrome type was treated with chaihulonggumuli decoction or banxia Baizhu Tianma decoction4.The prognosis of 62 patients with brain metastasis of lung cancer was analyzed by single factor analysis.The results showed that the survival time of patients with brain metastasis of lung cancer was related to pathological type,KPS score of initial diagnosis,EGFR-TKI three-generation targeted drug therapy,brain radiotherapy and high exposure to traditional Chinese medicine(p 0.05).Cox multivariate analysis showed that high exposure to traditional Chinese medicine,brain radiotherapy and KPS score were independent prognostic factors(p 0.05).Therefore,head radiotherapy,high exposure to traditional Chinese medicine and high initial KPS score are independent protective factors for prognosis of patients with brain metastases from lung cancer.Conclusion:1.The pathological classification and basic physical quality of patients with brain metastases from lung cancer are closely related to the prognosis of brain metastases from lung cancer,brain radiotherapy,EGFR-TKI three-generation targeted therapy,immunotherapy and high exposure to Chinese herbs can prolong the survival time of patients with brain metastases from lung cancer.2.In the treatment of brain metastases from metastatic lung cancer,TCM decoction can improve symptoms,improve quality of life and prolong survival.Different treatment methods of Western medicine can kill cancer cells,inhibit the growth of tumor,thus extending the survival period,in clinical treatment on the premise of standardization,the choice of individual treatment for patients. |
