Purpose:To explore the distribution characteristics of TCM syndromes and medication rules of patients with esophageal cancer,so as to provide supplement and reference for TCM clinical syndrome differentiation of esophageal cancer.Material and method:A retrospective study was conducted to collect the clinical data of patients with esophageal cancer in the Department of oncology,Affiliated Hospital of Liaoning University of traditional Chinese medicine from December 2016 to December 2020,to establish an information database,and to input the general situation,disease situation,anti-tumor treatment in western medicine,personal history,symptoms and signs,blood routine,Chinese herbal pieces and other relevant data of the patients SPSS 25.0 statistical software for frequency,frequency statistics.According to the diagnostic criteria of TCM syndrome differentiation,the clinical manifestations of patients were analyzed,the syndrome characteristics and medication rules of patients with esophageal cancer were summarized and analyzed,and the correlation between eight cardinal principles syndrome and peripheral blood inflammatory indexes was analyzed.Results:1.This retrospective study included 62 patients with esophageal cancer.There were 59 males and 3 females.The maximum age was 89 years old and the minimum age was 39 years old.The average age was 65.20 ± 9.15 years old,and the age was concentrated in 60-69 years old.2.The pathological types of 62 cases of esophageal cancer: 45 cases of squamous cell carcinoma,2 cases of adenocarcinoma,1 case of small cell carcinoma,1 case of adenosquamous carcinoma,11 cases of undetermined type,2 cases of no pathology.Among them,18 cases underwent operation,5 cases underwent esophageal stent implantation,28 cases received chemotherapy,18 cases received radiotherapy,and 15 cases received radiotherapy combined with chemotherapy.3.The common symptoms and signs of 62 patients with esophageal cancer were fatigue,obstruction of swallowing,emaciation,constipation,red tongue,thin and white fur.The more common symptoms and signs were less complexion,poor sleep,less food,poor appetite,vomiting,chest pain,abdominal distension,less urine,cough,cough,reddish tongue,white and greasy fur,weak pulse,slippery pulse,stringy pulse,etc.4.The clinical manifestations of 62 cases of esophageal cancer were analyzed according to the syndrome differentiation standard.There were 9 cases of cold syndrome(14.52%),27 cases of heat syndrome(43.54%),26 cases of no obvious cold and heat syndrome(41.94%),35 cases of deficiency syndrome(56.45%),11 cases of excess syndrome(17.74%),and 16 cases of deficiency and excess syndrome(25.81%).According to the number of cases,there were15 cases of deficiency heat syndrome(24.19%),8 cases of deficiency excess syndrome(12.90%),7 cases of deficiency excess syndrome(11.29%),8 cases of deficiency cold syndrome(12.90%),5 cases of excess heat syndrome(8.06%),and 1 case of deficiency excess syndrome(1.61%).5.Syndrome differentiation of TCM syndrome elements in 62 cases of esophageal cancer: 26 cases(41.93%)of qi deficiency syndrome,19 cases(39.65%)of yin deficiency syndrome,9 cases(14.52%)of Yang deficiency syndrome,11 cases(17.74%)of blood stasis syndrome,14 cases(22.58%)of phlegm dampness syndrome,5 cases(8.06%)of heat toxin syndrome and 6 cases(9.68%)of qi stagnation syndrome.There were 8 syndrome types in the combination of syndrome elements,including 15 cases(24.19%)of Qi and yin deficiency syndrome,12 cases(19.35%)of spleen and stomach weakness syndrome,8 cases(12.90%)of qi deficiency and phlegm dampness syndrome,3 cases(4.85%)of Jin deficiency and heat stagnation syndrome,9 cases(14.52%)of qi deficiency and Yang Micro syndrome,6 cases(9.68%)of phlegm and blood stasis syndrome,5 cases(8.06%)of heat toxin and blood stasis syndrome,and 4 cases(6.45%)of yin deficiency and heat toxin syndrome.6.By single factor analysis of variance,NLR,LMR,PLR,red blood cell count in the distribution of heat syndrome,cold syndrome,no obvious cold and heat syndrome had statistical difference(P < 0.05).There was a certain correlation between heat syndrome and NLR,LMR,PLR,red blood cell count.Logistic regression analysis showed that when NLR ratio increased,the risk of heat syndrome was higher.NLR in heat syndrome and cold syndrome,no obvious cold and heat syndrome between pairwise comparison,significant difference(P < 0.05),in cold syndrome and no obvious cold and heat syndrome pairwise comparison,no significant difference(P > 0.05).7.36 patients were treated with traditional Chinese medicine decoction.The basic prescriptions used more than 10 times were Xiangsha Liujunzi Decoction,Chaihu Longmu Decoction and Shashen Maidong decoction.Conclusion:1.The sym Ptoms of Patients with eso Phageal cancer are com Plex,mainly with deficiency syndrome,es Pecially with deficiency of qi and deficiency of Yin.There are many kinds of sym Ptoms and signs.2.The eight principles of syndrome differentiation is applicable to the traditional Chinese medicine syndrome differentiation of esophageal cancer.Cold,heat,deficiency and reality are the key principles,which can be combined with syndrome elements.3.The TCM syndromes of middle and advanced esophageal cancer patients generally tend to deficiency syndrome and heat syndrome,of which deficiency-heat syndrome and deficiency-excess syndrome are more common.4.NLR is an independent factor influencing TCM syndrome type of esophageal cancer.When NLR ratio increases,the risk of TCM syndrome type developing to heat syndrome is greater.5.The traditional Chinese medicine treatment of esophageal cancer is combined with strengthening the body and eliminating pathogenic factors,with Liujunzi Decoction,Xiaochaihu Decoction and Shashen Maidong decoction as the basic prescriptions.
