A Retrospective Study Of Chi's Closed Reduction Combined With PFNA In The Treatment Of Intertrochanteric Fractures

Objective: To retrospectively analyze the clinical effect of closed reduction combined with PFNA in the treatment of intertrochanteric fracture.Materials and methods: A total of 173 patients with intertroteric fracture of femur were treated in the First Department of Orthopedics of Fushun Hospital of Traditional Chinese Medicine during January,2018 to December,2019.According to the inclusion criteria and exclusion criteria,44 patients treated with closed reduction of Chi bone setting combined with PFNA after admission were selected as the observation group,and 43 patients treated with traction bed combined with PFNA were selected as the control group.A total of87 patients,including 38 males and 49 females,were retrospectively analyzed.The causes of injury were different in each patient,which could be divided into three categories: 2 cases of falling from high place,20 cases of traffic accident,and 65 cases of slipping and falling on the ground.Type according to the principle of AO classification,A1,A3,A2 type 46 cases(16 cases,compared two groups of patients operation time,blood loss,preoperative reset when the number of C arm fluoroscopy,fracture healing time,complications,and postoperative day 3,5,7,14 days of phantom pain,the last follow-up of limb function grading,joint mobility,Statistical analysis was performed.Results: There was no statistical difference in clinical data between the two groups(P>0.05).After discharge,87 patients were regularly followed up for 12 months,and the incisions were all healed in stage I,without any cases of incision infection.1.The operative time,intraoperative blood loss and the number of C-arm fluoroscopy in preoperative reduction in the observation group were shorter(less)than those in the control group(P<0.05),and the postoperative complications and fracture healing time in the two groups were not statistically significant(P>0.05).2.The pain of the observation group was better than that of the control group on the day and the third and fifth days after surgery(P<0.05),which was statistically significant;On the 7th and 14 th day after surgery,the wound of the affected limb tended to heal,and there was no significant difference in pain between the two groups(P>0.05),without statistical significance.3.The hip function was significantly improved in the last postoperative follow-up between the two groups,and there was no difference in hip function and range of motion in the last postoperative follow-up between the two groups(P>0.05).Conclusion:Chi Shi bonesetting intertrochanteric fractures treated by closed reduction and PFNA can reduce the operation time,intraoperative blood loss and preoperative reset number of C arm fluoroscopy,effectively avoid preoperative of fracture end due to adjusting the reset when the secondary injury and the surrounding soft tissue injuries,reduce postoperative pain,decrease The Times of patients in the X line exposure and accelerate the early fracture healing,It is better than traction bed traction reduction combined with PFNA treatment,and has a certain value of promotion.
