Purpose:To explore the related risk factors of type 2 diabetes with non-alcoholic fatty liver disease,and analyze the pharmacological mechanism of Huang Lian in the treatment of type 2 diabetes with non-alcoholic fatty liver based on network pharmacology,aiming to reveal the etiology and pathogenesis of the two diseases,and provide new treatment ideas and reference basis for the treatment of type 2 diabetes combined with non-alcoholic fatty liver disease in the future with integrated traditional Chinese and western medicine.Methods1.A total of 107 patients who were hospitalized in the Endocrinology Department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from August2019 to May 2020 were collected,and their cases were retrospectively analyzed.According to the presence or absence of non-alcoholic fatty liver disease,they were divided into two groups.Record the medical record number,name,gender,age,waist circumference(WC),height(H),weight(W),course of disease,and calculate their BMI(BMI=W/H2).Collect patient's venous plasma and detect their FBG,2h PG,Hb A1 C,FC-P,2Hc-P,TG,virus hepatitis detection and fasting liver color Doppler ultrasound examination.Calculating the HOMA2-IR index.Multivariate Logistic regression was used to analyze related risk factors.2.Obtain the main chemical components and targets of Huang Lian through the TCMSP database,screen the active components according to ADME,and obtain the disease targets of type 2 diabetes combined with non-alcoholic fatty liver disease through the Gene Cards,OMIM,TTD,and DRUGBANK databases.Use STRING platform for protein interaction analysis,construct PPI network and explore potential protein function modules in the network.Use R 3.6.3 software to reference related data packages for GO function enrichment analysis and KEGG pathway enrichment analysis.The Cyto Scape 3.7.1 software was used to construct a network of "drug ingredients-target-pathways".Results:1.Comparing the physical and chemical indexes of the type 2 diabetes mellitus combined with non-alcoholic fatty liver group and the non-fatty liver group,the BMI,HOMA2-IR(CP),WC,TC,TG,FCP ofthe T2 DM combined NALFD group were significantly higher than those of the simple T2 DM group(P<0.05).There was no significant difference between the two groups in FBG,2h BG,HDL-C,LDL-C,ALT,AST,2h CP,Hb A1c(P>0.05).2.Through multivariate logistic regression analysis,TG,BMI and HOMA2-IR are the main risk factors for NAFLD.3.The core active ingredients of Huang Lian in regulating type 2 diabetes and non-alcoholic fatty liver disease are quercetin,berberine,berberrubine,etc.The core targets are IL-6,TNF,RELA,IL1 B,etc.The main biological components are the pathway acts on the AGE-RAGE signaling pathway in diabetic complications,TNF signaling pathway,IL-17 signaling pathway,HIF-1 signaling pathway,etc.Its main function is to reduce fat production by inhibiting the fluid shear stress and atherosclerosis signaling pathway.Conclusion:1.Through a retrospective study of 107 patients,the major risk factors for the development and development of type 2 diabetes with non-alcoholic fatty liver disease are insulin resistance,dyslipidemia,and obesity.2.This study first revealed a multi-component,multi-target,and multi-pathological mechanism of Coptis chinensis in the treatment of type 2 diabetes with non-alcoholic fatty liver disease.Network pharmacology predicts that quercetin is Coptis chinensis.Biological processes such as the therapeutic component of the disease,mainly the reaction to lipopolysaccharide and the process of reactive oxygen metabolism,are involved,and targets such as IL6,TNF and RELA are involved in the regulatory process.The above results will be useful in the future.The development and use of Coptis chinensis in type 2diabetes with non-alcoholic fatty liver disease provides a theoretical basis.
