Objective:acute renal injury is a group of heterogeneous diseases characterized by a sudden decrease in glomerular filtration rate,characterized by increased serum creatinine or oliguria,and classified by stage and cause.After high-intensity exhaustive exercises such as marathon and Triathlon,the incidence of acute kidney injury in athletes is higher.It has been found that the expression of proinflammatory cytokines is extremely active after exhaustive exercise,which may be one of the inducements of acute renal injury.According to the abnormal expression of cytokines in acute renal injury,the animal model of acute renal injury in Wistar rats was established by one-time exhaustive treadmill exercise and the intervention experiment of TNF-? inhibitor was carried out to explore the effect of TNF-?/NF-?B signal pathway mediated inflammation after exhaustive exercise and the intervention effect of TNF-? inhibitor on acute renal injury induced by one-time exhaustive exercise.To provide experimental and theoretical basis for reducing and preventing the occurrence of acute renal injury.Research Method:Thirty male Wistar rats(8 week age,weight 200-220g)were randomly divided into three groups:control group(EC group,n=10),exhaustive exercise group(EE group,n=10),exhaustive exercise combined with TNF-? inhibitor intervention group(TEE group,n=10).The TEE group was orally injected with TNF-? inhibitor(thalidomide)200mg/kg body weight 4 hours before each exercise,and thalidomide was dissolved in 0.9% normal saline at a concentration of 40mg/ml.The rat model of acute renal injury was established by running on the treadmill to exhaustion.Serum creatinine(Cr)and urea nitrogen(BUN)were detected by sarcosine oxidase method and urease method respectively;Determination of tumor Necrosis Factor-?(TNF-?)and Interleukin-1?(IL-1?)in Serum of Rats in three Group by enzyme-linked Immunosorbent Assay;The pathological changes of renal tissue of rats in each group were observed by light microscope;The expressions of nuclear factor-?B(NF-?B),TNF-? and IL-1? in kidney tissue of rats in each group were detected by Western blotting.Result:1.After one-time exhaustive exercise,the contents of serum BUN and Cr in EE group increased significantly(P<0.01),and the renal tissue sections of EE group showed glomerulus swelling,renal tubular epithelial cell exfoliation and inflammatory cell infiltration.2.Compared with EC group,the serum inflammatory cytokines TNF-? and IL-1? in EE group increased significantly(P<0.01),and the protein expression of TNF-?,NF-?B and IL-1? in renal tissue increased significantly(P < 0.05).3.Compared with EE group,the contents of serum BUN and Cr,the serum inflammatory factors TNF-? and IL-1? in TEE group were significantly decreased(P< 0.05),and the expression of TNF-? and NF-?B protein in renal tissue was significantly lower than that in EE group(P < 0.05).The degree of renal tissue injury in TEE group was less than that in EE group.Conclusion:1.After one-time exhaustive exercise,the rat model of acute renal injury was successfully established with renal dysfunction and renal morphological injury.The occurrence of acute kidney injury may cause TNF-? and NF-?B-mediated inflammatory signaling pathway induces inflammation and further damages renal tissue.2.Thalidomide may act by inhibiting the proinflammatory cytokine TNF-?generation and further inhibition activation of NF-?B protein alleviates renal dysfunction and alleviates the inflammatory effects caused by acute kidney injury after exhaustive exercise.
