Research On The Regularity Of Acupoint Selection In Different Periods Of Acupuncture Intervention For Ischemic Stroke Based On Data Mining Technology

ObjectiveTo explore the rules of acupuncture treatment prescription in different periods after ischemic stroke by data mining method.MethodsSearch for the clinical research literatures about acupuncture treatment of ischemic stroke issued on CNKI database from the database established up to December 31st,2020,and the information of different period(acute stage,recovery stage,sequelae stage),subjects' symptoms,treatment plan,etc.,were extracted,and the database was established for data mining.Frequency statistics were completed in Excel,and Gephi was used for visually presentation of the frequency statistics results on each period.SPSS Modeler was used for association analysis and complex network diagram prepared.Cytoscape was used to make and beautify the co-occurrence network map.SPSS was used for systematic cluster analysis.All these methods were to explore the rules of acupuncture treatment different periods of ischemic stroke.Results1.Literature inclusionA total of 870 literatures(976 groups)with 49,628 patients were included in the acute stage,541 literatures(619 groups)in the recovery stage,with 27,805 patients,and 67 literatures(72 groups)in the sequelae stage,with 4201 patients.2.High frequency acupointsAmong all the stages after ischemia stroke,Zusanli,Hegu,Sanyinjiao,Quchi and Neiguan are the five most commonly used acupoints.40 high-frequency acupoints were used in all the three phase,which basically cover main acupoints used in clinical treatment of cerebral ischemia with acupuncture.The high frequency acupoints in the acute phase and the recovery phase coincide more.The acupoints in the sequelae phase are included in the acute phase or the recovery period.The first 10 high-frequency acupoints of all the three phases:Zusanli,Hegu,Sanyinjiao,Quchi,Neiguan,Yanglingquan,Waiguan,Jianyu,Taichong,Shuigou.In addition to the above-mentioned common high-frequency acupoints,the top 10 high-frequency acupoints in the acute phase:Siqiang,Touwei,Shaohai,Laogong,Mingmen,Waijinjin,Houding,Waiyuye,Jianshu,Qianding;The first 10 high-frequency acupoints in the recovery period:Tianzong,Jianjing,Quze,ouch point,Jianqian,Quepen,Tianquan,Benshen,Zhiqiang,Chengfu;First 10 high-frequency acupoints of the acute period&recovery period:Biguan,Daling,Ganshu,Geshu,Guanyuan,Houxi,Jiache,Jianzhen,Jinjin,Lianquan;Acute phase&sequelae phase high frequency acupoints:Lieque.3.High frequency meridiansIn each stage after stroke,acupoints of hand Yangming meridian is the most frequently used acupoint.The application of foot Yangming meridian in the acute stage is second only to the large intestine meridian of hand Yangming and the gallbladder meridian of foot Shaoyang,and the application of stomach meridian in the convalescence stage and the sequela stage is second only to the large intestine meridian.The application of foot Shaoyang meridian decreased with the passage of time.4.Complex network graph resultsStrong links of the acute period:Zusanli-Hegu,Hegu-Quchi,Zusanli-Quchi,Zusanli-Sanyinjiao,Hegu-Sanyinjiao.Strong links of the recovery period:Hegu-Quchi,Hegu-Zusanli,Zusanli-Quchi.Strong links of the Sequelae period:Quchi-Zusanli,Hegu-Zusanli,Sanyinjiao-Zusanli,Hegu-Quchi.5.Correlation analysis resultsThere were 285 association rules in the acute stage,241 rules in the recovery stage and 275 rules in the sequelae stage.Although the number of references included in each stage differed greatly,the number of associations was basically the same.6.Cluster analysis resultsIt's concluded that Xingnao Kaiqiao acupuncture,traditional body acupuncture,local acupoints on head,Tongdu Tiaoshen acupuncture,Quyu Shengxin acupuncture method were used.Among these,the apply of Xingnao Kaiqiao acupuncture,traditional body acupuncture and local acupoints of head were showed in the cluster analysis of all the three phases.The applying of Tongdu Tiaoshen acupuncture was obvious on recovery phase and Quyu Shengxin acupuncture was on both the acute stage and recovery stage.Conclusions1.There are numerous studies on acupuncture treatment of stroke,but the number of studies which distinguish hemorrhagic stroke from ischemic stroke and post-stroke stages is greatly reduced,especially the literature on sequelae stage is insufficient.2.The acupuncture treatment of ischemic stroke has a high degree of acupoint coincidence in each stage,and each stage has its own different acupoints respectively.The acupoint selection shows a changing rule with time going by after stroke.Hand Yangming meridian,food Yangming meridian,foot Shaoyang meridian and Governor vessel acupoints are used frequently.The use of foot Shaoyang meridian tends to decrease over time after stroke.Zusanli,Hegu,Sanyinjiao,Quchi,Neiguan points are the most used 5 acupoints in all the three phase.besides,there are high frequency acupoints of each phase respectively,for Siqiang,Touwei,Shaohai,Laogong,Mingmen of acute phase,Tianzong,Jianjing,Quze,ouch point and Jianqian of recovery phase,Biguan,Daling,Ganshu,Geshu,Guanyuan,Houxi of acute meridian&recovery meridian and Lieque point of acute phase&sequelae phase.3.As to the rule of acupoints combination,selection of local points,selection of corresponding channel point,selection of same-name channel points,superior-inferior points combination and Five-Shu points are the regular acupoint combination method and they were all applied among the acute phase and sequelae phase.In recovery phase,selection of local points,selection of corresponding point,selection of same-name channel points and Five-Shu points were used.In the method of acupoint combination,there is little difference among the three stages.4.Xingnao Kaiqiao acupuncture,traditional body acupuncture,local points of head,Tongdu Tiaoshen acupuncture and Quyu Shengxin acupuncture are the common methods currently used in ischemic stroke's acupuncture treatment,.Among these,the apply of Xingnao Kaiqiao acupuncture,traditional body acupuncture and local points of head are applied frequently in all the three phases whereas Tongdu Tiaoshen acupuncture in recovery phase and Quyu Shengxin acupuncture in both the acute stage and recovery stage.
