Professor Xu Xin's Experience In The Treatment Of PCOS-IR With Prescription And Syndrome

Background:Polycystic Ovary Syndrome(PCOS)is the most common endocrine and metabolic disorder in gynecology.Its main clinical manifestations include irregular menstruation,abnormal uterine bleeding,amenorrhea,anovulatory infertility,obesity,abnormal glucose and lipid metabolism,hirsutism,acne and so on.A large number of studies have found that PCOS not only has significant effects on female reproduction,but also may have significant effects on glucose and lipid metabolism,cardiovascular,endometrium,breast,skin,bone and psychology in women.At present,the treatment of PCOS-IR by western medicine is mostly treated with hormone drugs or surgical treatment,and adverse reactions are more likely to occur,which has a certain impact on the quality of life of patients.Traditional Chinese medicine treatment of PCOS-IR has unique advantages,starting the renal qi-tiangui-chongren-uterine reproductive axis,stimulating the vital qi,restoring menstruation,and maintaining women's life and health in a longer termObjective:Based on the real outpatient medical records,the rule of prescription medication can be analyzed to explore the treatment characteristics of polycystic ovary syndromes-insulin resistance(PCOS-IR).Meanwhile,the efficacy of TCM treatment of PCOS-IR is analyzed,and the age characteristics,syndrome type distribution,hormone and B-ultrasound distribution of patients with PCOS-IR,PCOS and IR is exploried.It provides ideas for clinical treatment of PCOS-IR.Methods:The medical records of PCOS-IR patients treated by Professor Xu Xin of Department of Gynology in Beijing Hospital of Traditional Chinese Medicine from January 2017 to April 2021 were collected and divided into 3 groups.The association analysis method was used.Data mining software was used to count the high-frequency single drug,drug group and associated drugs in each group,and analyze the regularity of taste return meridian and efficacy,so as to explore the characteristics of drug use in the three groups.Patients meeting the diagnostic criteria of PCOS,PCOS-IR and IR were treated with "Kidney-tonifying and Turbidity Removing Prestition",and treatment was added or reduced according to the therapeutic effect.One month was set as one course of treatment.Due to different patient conditions and compliance,different courses of treatment were taken.The shortest course of treatment of 1 course and the longest course of treatment of 24 courses were selected for statistical efficacy statistics,and statistical indicators included menstrual cycle recovery rate,BBT,dominant follicles under B-ultrasound,ovarian size,FSH,LH,FINS,BMI and other indicatorsResults:A total of 43 patients with PCOS-IR,37 patients with PCOS,and 10 patients with IR alone were collected.A total of 671 consultations were performed on these patients before and after treatment,involving 119 herbal medicines.These drugs mainly include deficiency medicine and heat-clearing medicine,with flat or cold properties,and mainly sweet and bitter taste.The meridian is mainly composed of liver and kidney,spleen and stomach,and large intestine.Through cluster analysis,five commonly used drug combinations were summarized,namely,the group of regulating qi-flowing and clearing heat:fried fructus gardeniae+pericarpium citri reticulatae+fructus aurantii+oyster shell;Qingtonifying group for stagnation and stasis removal:radices glehniae+flos mume+Chinese waxgourd peel+semen persicae;Tonifying Kidney and spleen Recuperation Group:fried liquorice+loranthus+radices dipsaci;Spleen and Remove turbid pathogenic to ngfu group:atrina glass+largehead atractylodes rhizome+coix seed+dodder seed;Clearing and reducing turbidity and regulating menstruation group:Indian bread+fruit of glossy privet+Chinese Thorowax+gypsum+baical skullcap root+nacre+tuber fleeceflower stem+hirsute shiny bugleweed herb+inner membrane of chicken gizzard+common selfheal fruit-spike+carbonized india madder root+peony root.In the PCOS-IR group,7 cases were cured,8 cases were significantly effective,23 cases were effective and 5 cases were ineffective.The recovery rate was 34.9%.The total effective rate was 88.37%.The pregnancy rate was 16.7%.In the PCOS group,10 cases were cured,15 cases were significantly effective,6 cases were effective and 6 cases were ineffective.The recovery rate was 67.6%.The total effective rate was 83.8%.The pregnancy rate was 11.1%.In the IR group,3 cases were cured,2 cases were significantly effective,4 cases were effective,and 1 case was ineffective.The callus penetrance was 50%.The total effective rate is 90%Conclusion:Professor Xu Xin applied " Kidney-tonifying and Turbidity Removing Prestition " to treat PCOS-IR with definite curative effect.It can invigorate the spleen and kidney,clear the turbidity and heat,smooth the liver and regulate qi,harmonize the chong and ren,restart the mechanism of tiangui,and achieve the purpose of curing the disease.
