| Heart failure is a common disease worldwide.Global data show that since the 21st century,the impact of heart failure on humans has been increasing worldwide.In different countries and regions,the causes of heart failure are not the same.More and more evidences show that the beginning of heart failure means high morbidity and high mortality.The prevention of heart failure on a global scale should be the focus of today’s main treatment.As a common and complex disease,heart failure has seriously affected people’s quality of life and medical and health systems,and the prevention and treatment of this disease is imminent.The in-depth research of Chinese medicine has brought new highlights to the research progress of heart failure.Chinese medicine has played a unique advantage in the prevention and treatment of chronic heart failure,which can effectively improve exercise tolerance,improve the quality of life of patients,and delay life time.As a non-invasive method that can be used to monitor cardiovascular health,it is easy to use,saves time and cost,and can predict the cyclic events of the heart.Advances in technology have made the use of pulse cardiogram to monitor cardiovascular health events become may.Traditional Chinese medicine has gradually played its unique advantages in improving the clinical symptoms of patients with chronic heart failure,improving the quality of life of patients,and prolonging life time.It has gradually become a hot topic of research by Chinese medicine personnel in recent years.Previous studies have shown that pulse cardiogram is related to changes in TCM syndromes of chronic heart failure,and the indicators of pulse cardiogram can reflect the severity and prognosis of chronic heart failure.Based on the former research,we will further explore the TCM syndromes of chronic heart failure and heartbeat indicators,and the characteristics and changing laws of gender and heartbeat indicators.Objectives:(1)Evaluate the difference in the amplitude of the main indicators H,I,J,and K waves of the shock cardiogram and the time interval between peaks tHI,tIJ,tJK between different TCM syndrome types in the chronic heart failure group;(2)To study the NT-proBNP level,left ventricular ejection fraction(LVEF),left ventricular fractional shortening(LVFS),left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVFS),left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD)The difference between the LVESD(Ventricular End-systolic Diameter);(3)To study the correlation between the main indicators of the heart failure group(H,I,J,K,tHI,tIJ,tJK)and gender.Methods:The study included 81 patients admitted to the General Ward and Intensive Care Unit of Beijing University of Chinese Medicine from September to February 2020,including 81 heart failure patients who were admitted according to inclusion criteria and exclusion criteria.The diagnostic criteria of chronic heart failure in Western medicine refer to the diagnostic criteria of 2016 ESC guidelines for the diagnosis and treatment of acute and chronic heart failure and the Chinese guidelines for the diagnosis and treatment of heart failure 2018,the diagnostic criteria of TCM syndrome types of heart failure refer to the standard of TCM syndrome types of chronic heart failure in 2014,the three basic syndrome types of Qi deficiency and blood stasis syndrome,Qi Yin deficiency and blood stasis syndrome,Yang Qi deficiency and blood stasis syndrome were selected as the main observation indexes.At least one deputy director and more than one clinician will make a Western medicine diagnosis to the inpatients,and for the patients who are diagnosed with heart failure,two doctors with the title of "Associate Director of traditional Chinese medicine or integrated traditional Chinese and western medicine" or above were assigned to make independent diagnosis and syndrome differentiation.General Information,laboratory examination,results of impact echocardiography and echocardiographic parameters were recorded and collected.After finishing the statistical analysis,all will be handled by SPSS 22.0 statistical software.Results:1.J value has statistical significance among the three TCM syndrome types of chronic heart failure.With the changes of Qi deficiency and blood stasis syndrome,Qi Yin deficiency and blood stasis syndrome and yang qi deficiency and blood stasis syndrome,the J value showed a decreasing trend.There was significant difference between the two groups in qi-yin deficiency and blood stasis syndrome and yang-qi deficiency and blood stasis syndrome.There was no difference in the indexes of yu-xin impact diagram between TCM syndrome types.2.Sex was correlated with H value and tIJ value.The values of H and Tij are higher in men than in women.3.NT-proBNP,cardiac function classification,LVEF,LVFS and LVEDD were different in chronic heart failure.NT-proBNP only has difference between Qi deficiency and blood stasis syndrome and yang qi deficiency and blood stasis syndrome.With the changes of Qi Yin deficiency and blood stasis syndrome,NT-proBNP increases gradually The difference of LVEF between qi-deficiency and blood-stasis syndrome and yang-qi-deficiency and blood-stasis syndrome was found in the classification of cardiac function,and the LVEF decreased with the changes of qi-deficiency and blood-stasis syndrome,qi-yin deficiency and blood-stasis syndrome and yang-qi deficiency and blood-stasis syndrome.LVFS was different between the two comparisons.The Order of change of LVFS value is qi-yin deficiency and blood stasis syndrome<qi-deficiency and blood stasis syndrome<Yang-qi deficiency and blood stasis syndrome;With the changes of Qi deficiency and blood stasis syndrome,Qi Yin deficiency and blood stasis syndrome,Yang Qi deficiency and blood stasis syndrome gradually increase.Conclusions:1.The J value can indirectly reflect the severity of chronic heart failure.2.There were significant differences in H value and tIJ value between male and female.3.J value,NT-proBNP,LVEF,LVFS,LVEDD,LVEDD and the classification of heart function were correlated with the syndrome types of chronic heart failure. |
