Analysis Of The Curative Effect Of Sanyu Zhitong Ointment On Early Swelling Of Closed Fractures Of Extremities

Objective : This study observed and compared the therapeutic effect of mannitol alone and combined application of Sanyu Zhitong Ointment and mannitol on early swelling of closed fracture of extremities,evaluated the clinical effect of Sanyu Zhitong Ointment,and provided theoretical basis and clinical basis for its clinical promotion.Methods :The clinical cases of swelling after closed fracture of extremities hospitalized in the Department of orthopedics,Wu Yi Hospital of traditional Chinese medicine,Jiangmen City from November,2019 to November,2020 were included in this study.The total number of cases was 60.The patients were randomly divided into mannitol group(hereinafter referred to as control group)and mannitol plus Sanyu Zhitong ointment group(hereinafter referred to as experimental group),30 cases in each group.Control group: after basic treatment,20% mannitol was given intravenously on the first day after admission,once a day for 7 consecutive days.Experimental group: on the basis of the control group,apply Sanyu Zhitong ointment on the affected area for 4 hours,once a day,for 7 days.The total course of treatment was 7 days,and the initial record was the day of treatment.At the same time(15:00)on the 1st,3rd,5th and 7th day after treatment,the swelling degree,skin temperature and the time of first appearance of dermatoglyphics were recorded;VAS scores were recorded before sleep(21:00)on the 1st,3rd,5th and 7th day after treatment;The levels of IL-6 and ESR were detected before and after treatment.All the above data were analyzed by SPSS 22.0.Results :(1)There was no statistical significance in general information(gender,age,injury site,injury factors,fracture AO type)between the two groups(P>0.05).(2)There was no significant difference in the degree of swelling between the two groups on the first day after treatment(P>0.05).On day 3,5 and 7 after treatment,the swelling degree of experimental group was lower than that of control group,and the difference was statistically significant(P < 0.05).(3)There was no significant difference in skin temperature between the two groups on the 1st and 3rd day after treatment(P>0.05).On the 5th and7 th day after treatment,the difference was statistically significant(P <0.05).(4)There was statistically significant difference in the time of the first appearance of skin stripes(P < 0.05).(5)There was no significant difference in VAS score between 2 groups on the 1st and 3rd day after treatment(P>0.05);On day 5 and 7,the difference was statistically significant(P < 0.05).(6)In terms of inflammatory indexes,there were no statistically significant differences in the levels of IL-6 and ESR before treatment(P>0.05),but the levels of IL-6 and ESR were all decreased after treatment,with statistically significant differences(P <0.05).(7)Liver and kidney function indexes: The liver and kidney function indexes of the two groups before and after treatment were within the normal range,without adverse reactions.Conclusion :(1)Mannitol group and mannitol plus Sanyu Zhitong ointment group can eliminate swelling,reduce pain and inhibit inflammatory reaction in the early stage of closed fracture of extremities to a certain extent after the total course of treatment.(2)The effect of mannitol plus Sanyu Zhitong ointment group was better than mannitol group in eliminating swelling,reducing pain and inhibiting inflammatory reaction after the total course of treatment.(3)Mannitol combined with Sanyu Zhitong ointment can improve the early swelling of closed fracture of extremities with significant effect and high safety,which is worthy of further clinical application.
