Research On The Division And Cooperation Mechanism Of Medical Institutions In Regional Medical Consortia

Research Purpose:It is one of the key work of health care reform that through the establishment of hierarchical medical treatment system,to establish a reasonable and effective medical pattern,so that to ease the status of "it's too difficult and expensive to see a doctor".And regional medical alliance is considered an important carrier to promote hierarchical treatment.This paper studies the role of medical institutions at all levels in the regional medical alliance and how to strengthen the division of labor of them.On the one hand,provide reasonable suggestions for the efficient operation of regional medical alliance,to promote the return of medical institutions at all levels of their own functional standards,and to play their own social and economic benefits at the greatest extent,as well to improve the efficiency of health resources utilization.On the other hand,through establish a division of labor mechanism at all levels of medical institutions to promote hierarchical medical treatment around the country to provide reference.Research Methods:First,through the literature research and interviews with managers of regional medical alliance management Office and other forms to have a preliminary understanding of the operation of a regional medical alliance in Beijing.Later,combined with pre-research results to carry out field research,questionnaires,in-depth interviews and seminars,focusing on the collection of the division of labor at all levels of medical institutions to carry out the status quo in Regional medical alliance,to explore and analyze its effectiveness and problems,and put forward countermeasures and suggestions.Research Content:Combing the development of regional medical alliance,main mode and implementation status quo,achievements,the existence of the problem of medical institutions at all levels of division of labor of the different models in China,and analysis the operation mechanism and system guarantee of the division of labor and cooperation of medical institutions in foreign regional medical alliance.Taking a regional medical alliance in Beijing as an object,understanding the status and effectiveness of medical institutions division of labor in medical alliance,analyzing and exploring the problems and causes of the medical institutions in the process of division of labor.Learning from the experience of the establishment of division of labor mechanism of medical institutions in the foreign regional medical alliance,and combined with China's national conditions and medical status quo of medical institutions at all levels to analysis of the role of medical institutions at all levels should plays.Clear their respective division of labor and duty obligations,explore the medical institutions at all levels of how to work closely in regional medical alliance,and find the best place to collaborate at all levels of medical institutions,so that to provide theoretical support and empirical basis for the establishment of coordination mechanism and management mode.Research Conclusion:The regional medical alliance at all levels of medical institutions to carry out division of labor has achieved some success.Such as medical institutions at all levels have the trends to gradually return to their respective functional positioning,the two-way referral channel has been initially get through,tertiary hospital medical service efficiency has improved,grass-roots community health service center's service capacity has improved,and grass-roots medical staffs' and patients' satisfaction have been improved.But there are still some problems,functional orientation at all levels of medical institutions has not yet fully returned,second-level hospital participation in collaboration is not tight enough,two-way referral channels are not smooth and so on.In this regard,the author explored the reasons for the existence of these problems and put forward some suggestions.First,we should adjust and further clarify the division of responsibilities of the medical institutions at all levels in medical alliance.Second,establishment of medical institutions at all levels of medical institutions collaboration mechanism,such as achieving medical synergy in the medical alliance,innovative personnel training and support mechanisms,establish a two-way referral constraint mechanism.Third,we must improve the relevant supporting measures to promote the division of labor of medical institutions to achieve,such as through giving the medical alliance full autonomy at the government level,and establish the medical institutions benefit distribution mechanism in the medical alliance to regulate the operation of the medical alliance.By increasing the financial investment for the medical alliance,and improve the medical insurance system to protect the operation of the medical alliance from the aspect of the economic.Strengthen the publicity of the medical alliance and so on.
