Effect Of Water Exercise Intervention On Lower Limb Motor Function Of Stroke Patients

Objective: In order to offer a scientific evidence for water exercise intervention of stroke patients,water exercise intervention was conducted in convalescent stroke patients to analyze the effect of water exercise intervention on lower limb motor function and explore the related mechanism.Methods: Thirty-four stroke patients aged 35 to 65 years were selected as experimental treatment subjects.Patients with similar functional scores were screened according to MMT score,standing balance function and inclusion and exclusion criteria.They were divided into water exercise intervention group and routine rehabilitation group,with 17 patients in each group.Routine rehabilitation and water exercise intervention + routine rehabilitation were performed for 8 weeks respectively.Before and after intervention,the range of motion,walking ability and balance function of the subjects were collected and evaluated respectively: Range of motion of joints(trunk flexion-extension Angle,torso left-right-lateral bending Angle,affected hip joint range,affected knee range),walking ability(step frequency,affected side step length,affected side touchdown time,weight-bearing symmetry,vertical center of gravity displacement),Balance function(open eye-motion track area,open eye-motion track length,closed eye-motion track area,closed eye-motion track length).Results:(1)Before exercise intervention,there was no significant difference in the range of motion of joints between the two groups(P>0.05).After 8 weeks of water exercise intervention,the trunk flexion and extension angle of the experimental group was 7.52±3.59,which was significantly reduced compared with the one before intervention(P<0.05).The range of motion of the left-right trunk flexion angle was 1.47±2.32,which was significantly increased than before intervention.The range of motion of hip joint and knee joint were38.55±9.10 and 33.88±12.43 respectively,which were significantly higher than those before intervention(P<0.01).The range of motion in routine rehabilitation group was improved,but there was no significant difference(P>0.05).After 8 weeks of water exercise intervention,the range of motion of the affected hip joint in the experimental group was 30.58±9.34 higher than that in the control group,and there was a significant difference(P<0.05).There was no significant difference in the range of motion of the affected knee joint between the two groups(P>0.05).(2)Before exercise intervention,there was no significant difference in walking ability test indexes between the two groups(P>0.05).After 8 weeks of water exercise intervention,the step length of affected side in the experimental group was 37.00±12.98,which was significantly increased compared with before intervention(P<0.01).In the experimental group,step frequency 0.46±0.19,affected side touchdown time 1.77±0.71,weight symmetry1.47±9.04 and center of gravity vertical displacement 1.55±0.88 were all improved,but there was no significant difference(P>0.05).There was no significant difference in walking ability test indexes between routine rehabilitation group and before intervention(P>0.05).After 8 weeks of water exercise intervention,there was no significant difference in the indicators of walking ability between the two groups(P>0.05).(3)Before exercise intervention,there was no significant difference in balance function test between the two groups(P>0.05).After 8 weeks of water exercise intervention,In the experimental group,the eye-opening-motion track area was 499.47±505.56,the eye-opening-motion track length was 485.29±164.70,the eye-closed moving-motion track area was 1317.29±879.51,and the eye-closed moving-motion track length was943.24±450.55,which were all reduced compared with before intervention.Significant difference was observed(P<0.01).In the control group,eye-opening-motion track area and eye-opening-motion track length were significantly lower than before intervention(P<0.01),and eye-closing motion track area was significantly lower than before intervention(P<0.05).After 8 weeks of water exercise intervention,there was no significant difference in balance function test indexes between the two groups(P>0.05).Conclusion: Both water exercise training and traditional rehabilitation training have positive effects on the improvement of lower limb dyskinesia of stroke patients,but the effect of water exercise is more obvious than conventional rehabilitation.The possible mechanisms are as follows:(1)The warm effect of water makes contracture muscles easier to stretch and reduces pain symptoms;(2)The buoyancy of water reduces the weight bearing of lower limbs,reduces the motion resistance of bone,muscle and connective tissue,and increases the range of motion of hip joint and knee joint,so as to improve the range of affected side step;(3)The anti-resistance training of water walking has greatly improved muscle strength and posture balance,reduced limb swing,and significantly reduced trunk flexion-extension Angle;(4)Hydrostatic pressure can stimulate tactile receptors and baroreceptors in the body,increase sensory input,and increase the activity of cortical sensory and motor areas,thus promoting sensorimotor integration.
