Integration Of Community Care Service For The Aged Care Service

With the aging of China’s population aging situation is getting deeper and deeper,the old people’s pension and medical rehabilitation problems become more prominent.Medical institutions and pension institutions,health and civil affairs departments belong to management,each independent system,when the change of elderly health status and selfcare ability,can not get timely and effective treatment,often have to return home,between hospitals and pension institutions,not only delay treatment,increase the cost,and to increase the burden on families.So the existence of "security" and "medical care" medical support combined with problems.In this paper,based on the theory of empowerment,community care theory and the theory of healthy aging,the research methods of literature analysis,interview and questionnaire survey were used in this paper.Analysis and verification of the community service for the elderly medical maintenance integration mode construction of the necessity and feasibility of supply at the current situation of Shanghai medicine maintenance integration,through the investigation found that the elderly to the degree of demand,then analyze and summarize the present old-age medical maintenance of the integration of China’s inadequate service mode of existence,and the reasons for these problems.Research and Analysis on the experience of medicine curing the integration of the development of different countries,and then put forward the construction of China’s medical maintenance integration of community service pension system based on then puts forward some countermeasures and suggestions for the integration of the system for maintenance of medicine.The paper believes that,based on the actual demand of healthy aging and the elderly as the center,home care service,community organizations and the coexistence of the three,and then from the four dimensions of policy,system,talent,ideas for strategic support and maintenance of medical integration pension service system suitable for our country.In order to complete the construction of this system need to promote medical care services and pension services for the development of mutual integration,integration of specific sound institutional mechanism to realize the health and pension resources,also need to improve the integration of medical care service maintenance and external conditions,including encouraging social forces to set up the combination of medical care institutions,strengthen the construction of talent team,and strengthen the Internet + "information support.
