Background: Human skin is the largest protective organ in the human body,and it also plays an important role in maintaining heat balance and metabolism of the human body.With the growth of age and environmental deterioration and other factors will affect skin texture,and then accelerate skin aging,and facial skin aging is especially concerned by people.The aging of facial skin is accompanied by complex biochemical processes with the increase of age,and the cellular structure of the skin at all levels is changed after aging,thus presenting the appearance of aging.Skin aging is mainly caused by heredity and external environmental stimulation.Heredity skin aging is an inevitable and natural physiological process determined by genes and generally starts at the age of 20.Skin aging caused by external environmental stimulation is mostly caused by the direct irradiation of the ultraviolet ray(UV)in the sun on the facial skin,which is also what we often say about skin photoaging,mainly manifested as the deepening and thickening of facial skin wrinkles,telangiectasia,freckles,pigmented spots,skin rough and loose,solar dermatitis and so on.With the development and progress of society and the continuous improvement of material and spiritual life,people’s demand for facial rejuvenation shows an explosive growth,and the anti-aging treatment methods for face are increasingly diversified,including surgical treatment,chemical stripping,laser treatment and injection of related equipment,and external drug treatment.Among them,injection is favored by people because of its minimally invasive treatment process,short recovery period and high comfort level.With the continuous emergence of new injection materials,platelet rich plasma(PRP)extracted from autologous venous blood has been widely used in recent years,and there are numerous reports that PRP is injected into the face with a syringe to improve facial wrinkles,pigmentation and so on to obtain good therapeutic effect.PRP has been used by clinicians in the medical industry for nearly 50 years.PRP is derived from human blood and is simple to prepare,it can be obtained by two differential centrifugation after taking venous blood of patients and adding anticoagulant.PRP can produce a large number of endogenous growth factors(GF),promote cell regeneration and tissue repair,promote the establishment of skin microcirculation,change the original skin stress system,smooth the skin fine lines and other functions.At the same time,compared with commercial injection filling products,PRP comes from autologous venous blood and does not produce immune rejection reaction with the human body,so it has better safety.Moreover,Platelets have stronger ability to promote tissue regeneration due to their physical and chemical properties.At present,PRP treatment of facial photoaging basically adopts syringe manual subcutaneous injection,microneedle roller introduction or high-pressure injection and so on,it is difficult to quantify the injection depth,injection dose and facial injection uniformity in all of the above problems,and the patients’ comfort level after treatment is poor due to the large injection needle and large single point injection dose.And to solve the above problem,it is necessary to objectively quantify the injection depth,dose of the syringe and the uniformity of facial injection,eliminate the above aspects depends only on subjective factors such as physicians’ own judgment and clinical experience,and can be achieved a precise and quantifiable treatment,greater safety,dangerous parts(such as eye week)can also be injected.The negative pressure electronic syringe with negative pressure suction technology can solve the above problems well,it can inject PRP at the depth set by doctors,using 32 G multi-needle microneedle injection,which greatly improves the efficiency.At the same time,the injection dose and injection frequency of each part can be accurately set,after the injection,the negative pressure head at the needle will release the pressure,and the injection material will not be wasted.This provides a more optimized treatment regimen for PRP injection therapy,making the treatment effect more significant.Objective: To observe the clinical effect of facial intradermal injection of autologous PRP using negative pressure electronic injector in the treatment of skin photoaging.Methods: From August 2020 to December 2021,32 patients admitted in our department for the treatment of facial photoaging who were suitable for PRP treatment were selected,the related characteristics of skin photoaging before and after treatment were used as a self-control study.The autologous venous blood of the enrolled patients was drawn to make PRP,and the PRP was injected intradermally on the face through an electronic injector,macroscopic and microscopic evaluation of skin-related features(pigmentation,texture,pores,telangiectasia,etc.)in different parts of the face using Photo MAX PRO dermoscope before and 3 weeks after treatment,at the same time,patients were asked to fill in the Face-Q scale for self-assessment.Results: Before and 3 weeks after treatment,Photo MAX PRO dermoscope was used to examine the skin-related characteristics of various parts of the face,and it was observed that the facial skin was firmed,the texture was reduced and more delicate,the complexion was brighter,pores were reduced,telangiectasia and pigmentation were reduced.The statistical results of the Face-Q scale showed that there were statistically significant differences in patients’ satisfaction with overall facial appearance and cheeks before and after the treatment.Most patients are satisfied in subjective satisfaction evaluation,and believe that treatment is worth the time and effort.Conclusion: Facial intradermal injection of autologous PRP can improve facial skin photoaging,and can significantly improve facial pigmentation,texture,pores,and telangiectasia.
