Objective: To develop a nomogram based on the MRI features and clinical-pathologic variables of breast cancer to predict pathologic complete response of breast cancer after neoadjuvant chemotherapy.Methods: 110 breast cancer patients who received neoadjuvant chemotherapy in the first hospital of Lanzhou University from April 2017 to April 2021 were considered for inclusion.MRI scans obtained before neoadjuvant chemotherapy were analyzed.According to the pathological evaluation system of neoadjuvant chemotherapy,all patients were divided into p CR group and np CR group,including p CR group(n = 31)and np CR group(n = 79).Univariate and multivariate logical regression analyses were performed to identify the independent variables associated to p CR.Multivariate logical regression analysis was performed to identify the statistically significant variables(P < 0.05)from which the nomogram was created.The nomogram was internally verified and calibrated by bootstrap resampling for1000 times.Nomogram discrimination ability was assessed with the receiver operating characteristic curve(ROC),its clinical application value was assessed with decision curve analysis(DCA).Results: Univariate logistic regression analysis showed that ER negative,PR negative,HER-2 positive,Ki-67 index high expression,molecular subtype,chemotherapy regimen,small tumor diameter and low breast edema classification were associated with p CR.Multivariate logical regression analysis results showed that PR negative,Ki-67 index high expression and low breast edema classification were independently associated with p CR.The nomogram showed good discrimination(AUC:0.85),predictive ability and clinical application value.Conclusion: The nomogram based on PR status,Ki-67 index and breast edema classification showed good discrimination,predictive ability and clinical application value in predicting breast cancer pathologic complete response after neoadjuvant chemotherapy.
