| Objective To explore the clinical application value of mp-MRI-based 3D model cognitive fusion in transperineal prostate targeted biopsy.Methods A total of 112 patients with indications for prostate biopsy and exclusion of contraindications were selected and randomly included in the experimental and control groups from January 2021 to December 2021 in the Department of Urology of the First People’s Hospital of Yunnan Province.50 patients in the experimental group,aged(69.14±8.24)years,had a median PSA of 13.40ng/ml,a median prostate volume of47.08 ml,and the number of needles was(10.80 ± 0.95).The median PSA was 11.73ng/ml,the median prostate volume was 37.00 ml,and the number of needles was(11.87± 1.14)in the control group of 62 patients aged(67.21 ± 8.92)years.All patients achieved mp-MRI examination.The raw image data of the experimental group were saved in DICM format and imported into the Materialise Mimics system to establish a3 D prostate model.In the experimental group,after cognitive fusion,a total of "10+X" stitches were performed for the targeted and system biopsy,where "X" was 1~3 stitches for the targeted cognitive fusion.In the control group,"12" stitches of TRUS system biopsy were performed.To evaluate the function of 3D models of the prostate in physician-patient communication and physician training through questionnaires for patients and young physicians;and to evaluate the diagnostic value of mp MRI-based3 D cognitive fusion in targeted prostate biopsy through pathology results.Results In this study,there were no significant differences in the indicators of demographic characteristics between the two groups of patients included(P > 0.05).The Pca positivity rate was 56.0% in the experimental group and 43.5% in the control group,which was higher in the former,but it was not statistically significant(P > 0.05);the cs Pca detection rate was higher in the experimental group than in the control group(38.0% vs 32.3%),but the difference was not statistically significant(P > 0.05);in terms of single needle positivity rate,the difference between the targeted group and the systemic group was statistically significant(P < 0.05).The most frequent complication in both groups was hematuria(18.0% vs.24.2%),followed by fever and acute urinary retention,with no significant difference in the occurrence of complications between the two groups(P> 0.05),and no serious complications were observed in either group.Compared with the 2D group,patients in the 3D group were significantly more satisfied with the anatomical structure,surgical purpose,and communication(P< 0.05).Physicians trained in the 3D group had a higher level of knowledge of anatomical structures and surgery compared to the 2D group(P < 0.05).Conclusion 1.mp MRI-based 3D cognitive fusion-guided transperineal biopsy can achieve the same diagnostic effect as systemic biopsy with fewer needles,and transperineal targeted biopsy is a safe and reliable puncture method.2.3D models of the prostate can improve patients’ cognition of their condition and their understanding of surgery,and it also can further improve young physicians’ understanding of prostate anatomy and surgical schedule,and have some value in communication between doctor-patient and training.3.3D technology may have some guidance value in the preoperative planning of prostate cognitive fusion biopsy. |
