Objective:To explore the clinical value of uric acid in the diagnosis of renal injury in gestational hypertension and the prediction of perinatal outcome.Methods:A total of 550 patients with gestational hypertension who were diagnosed and delivered in Anhui Maternal and Child Health Hospital from August 2019 to January 2021 were collected.According to the classification of gestational hypertension,they were divided into 242 cases of gestational hypertension group,161cases of preeclampsia group,147 cases of severe preeclampsia group,and 200 cases of normal pregnant women who delivered during the same period in Anhui Maternal and Child Health Hospital;The cases of hypertensive disorders during pregnancy were divided into kidney injury positive group 137 cases and negative group 413 cases according to the presence or absence of kidney injury;The cases of gestational hypertension were divided into normal uric acid group and hyperuric acid group according to serum uric acid level;The differences in serum Cys C and UA between the pregnancy-induced hypertension group and the normal pregnancy group,as well as the kidney injury positive group and the negative group,were compared.The pregnancy complications and perinatal outcomes were compared between the hyperuric acid group and the normal uric acid group.One-way analysis of variance was used for the general data such as age,height,weight,and other general information of pregnant women in each group,the comparison of serological indexes between the normal pregnancy group and the gestational hypertension group,and the perinatal outcome,univariate analysis was performed using the X~2test.Results:1.Cys C and UA in gestational hypertension group were significantly higher than those in normal pregnancy group,the difference was statistically significant(P<0.05).Cys C and UA in gestational hypertension group were compared:gestational hypertension group was the lowest,Followed by preeclampsia,the severe preeclampsia group was the highest,and the difference was statistically significant(P<0.05).Comparison of Cys C and UA in the kidney injury group:the positive group was higher than the negative group,and the difference was statistically significant(P<0.05).2.There were differences in the occurrence of placental abruption,positive renal function indexes and fetal growth restriction between the two groups,and the difference was statistically significant(P<0.05).However,there was no significant difference in the results of hypertensive emergency,abnormal cardiac function and liver function,and HELLP syndrome between the two groups(P>0.05).3.The average gestational age in the normal uric acid level group was 38.73±3.00weeks,which was greater than 37.91±2.81 weeks in the high uric acid level group,and P<0.05,the difference was statistically significant;the birth weight of the newborns in the normal uric acid level group was 3262.23±650.01 g,higher than the high uric acid group of 2971.31±809.45g,and P<0.001,the difference was statistically significant.4.There was no statistically significant difference between the two groups in terms of gender and serum uric acid level of neonates(P>0.05).It can be seen in the two groups of delivery methods that the cesarean section rate in the normal uric acid level group was significantly lower than that in the high uric acid level group.And in the high uric acid level group,the cesarean section rate was higher than the vaginal delivery rate,P<0.05,the difference was statistically significant.5.Serum uric acid level increases the risk of neonatal preterm birth,the results are different,and the results have significant statistical significance,P<0.001,the comparison results of the two groups,whether it is mild asphyxia or severe asphyxia,or There was no significant difference between intrauterine distress or perinatal mortality,and the difference was not statistically significant,P>0.05.Conclusions:Serum uric acid level can be used as a predictor of gestational hypertension,which may predict the progression of gestational hypertension by predicting renal function impairment in gestational hypertension.At the same time,the occurrence of placental abruption,fetal growth restriction,premature birth,etc.can be predicted and prevented according to the serum uric acid level.This provides a new idea for clinical diagnosis and treatment of gestational hypertension and for predicting neonatal birth outcomes,thereby improving the quality of life safety for mothers and infants.
