Study On The Efficacy Of CPC-PMMA And PMMA Bone Cement In The Treatment Of Osteoporotic Vertebral Compression Fractures

Objective The clinical effect of CPC-PMMA modified bone cement and PMMA bone cement was studied to provide evidence for treating OVCFs with CPC-PMMA modified bone cement,then explored a more appropriate application of bone cement that could reduce postoperative complications of PVP.Methods A total of 100 patients with OVCFs treated by PVP from January 2019 to December 2021 were included in this retrospective analysis.There were 19 males and 81 females.The age range from 64 to 81 years and the affected vertebral bodies were located in thoracic,thoracolumbar,and lumbar segments in 11 cases,74 cases,and 15 cases.According to the treatment,100 patients were divided into the PMMA bone cement group and the CPC-PMMA bone cement group.There was no significant difference in the general data after statistical analysis,which was comparable.There was no statistically significant difference in general data.Medical records,anteroposterior,lateral thoracic,lumbar X-ray,CT,MRI examination,preoperative and postoperative functional scale data were collected between the two groups.Results All 100 selected cases were followed up for at least half a year.Comparison of preoperative general data showed that there were no significant differences in gender,age,and responsible vertebral body segment between the two groups(P > 0.05).Comparison of intraoperative data showed that there was no significant difference in operative time between the two groups(P > 0.05),and the amount of bone cement used in the CPCPMMA group was significantly higher than PMMA group,the difference was statistically significant(P< 0.05).There was no significant difference in anterior margin height between the two groups(P>0.05).The height of the anterior margin of the injured vertebra in both groups was significantly improved after surgery(P < 0.05),but there was no significant difference between the two groups(P>0.05).The distribution of bone cement detected by intraoperative fluoroscopy and postoperative CT was significantly different between the two groups.There were 32 cases of unilateral distribution and 18 cases of bilateral distribution in the PMMA group,25 cases of unilateral distribution,and 25 cases of bilateral distribution in the CPC-PMMA group.There were significant differences in the diffusion of bone cement between the two groups(P<0.05),and the CPC-PMMA group obtained better diffusion than the PMMA group.There was no significant difference in VAS score and ODI score between the two groups before operation(P>0.05).VAS scores in the two groups were significantly improved after surgery compared with those before surgery(P<0.05),and VAS scores in the PMMA group were better than those in the CPC-PMMA group after surgery,the difference was statistically significant(P<0.05).There was no significant difference in VAS scores between the two groups 6months after surgery(P>0.05).There was no significant difference in preoperative ODI scores between the two groups(P>0.05).ODI scores in both groups were significantly improved after surgery(P<0.05),and there was no significant difference in ODI scores between the two groups after surgery and 6 months after surgery(P > 0.05).There was no significant difference in the probability of leakage between PMMA group and CPCPMMA group(P > 0.05).There was significant difference in the probability of adjacent vertebral fracture between PMMA group and CPC-PMMA group(P<0.05).There was no significant difference in Cobb Angle between the PMMA group and the CPC-PMMA group(P > 0.05).All bone cement leakage had no clinical symptoms,and no serious complications such as infection,embolism,or epidural hemorrhage occurred in all patients after surgery.Conclusion CPC-PMMA bone cement had the same clinical efficacy as PMMA bone cement and had an obvious immediate analgesic effect.Although more bone cement was used,a better dispersion effect could be obtained,and adjacent vertebral fractures were significantly reduced in the postoperative follow-up period.
