Correlation Of Tinnitus With Anxiety And Depression Before And After Cochlear Implant

Objective To report the correlation between tinnitus and anxiety and depression before and after cochlear implantation,as well as the related factors.Methods A total of 39 adult patients who received cochlear implantation and had tinnitus before surgery in the First Affiliated Hospital of Anhui Medical University from October 2019 to December 2020 were selected in this study.All patients independently completed the THI,HAMA and HAMD.And to analyze the tinnitus changes,anxiety and depression with different tinnitus degree before and after implantation,and the correlation between the tinnitus changes and anxiety and depression changes before and after operation as well as the influencing factors.Results Tinnitus was improved in 92% of patients after operation.THI scores was significantly reduced after implantation,and THI scores were significantly different before and after implantation（P < 0.001）.The degree of anxiety and depression increased with the increase of tinnitus severity before and after cochlear implantation.The severity of tinnitus was positively correlated with anxiety（r=0.925,P < 0.001）and depression（r=0.887,P < 0.001）before and after implantation.Among the three dimensions of tinnitus,emotionality was positively correlated with anxiety（r=0.880,P < 0.001）and depression（r=0.807,P < 0.001）.The change of mental factor in anxiety level（r=0.862,P< 0.001）and the change of anxiety/somatization factor in depression level（r=0.839,P <0.001）had the most positive correlation with the change of tinnitus severity.There was no significant difference between the severity of tinnitus and the changes of cognitive impairment,day-night change,tardiness and sense of hopelessness in depression before and after cochlear implantation（P > 0.05）.Conclusion For the candidates with tinnitus before surgery,cochlear implantation has a positive effect on tinnitus.And tinnitus is closely related to anxiety and depression,suggesting that physicians should pay proper attention to the mental health problems of tinnitus patients in clinical diagnosis and treatment.
