| OBJECTIVE:Multispectral refraction topography(MRT)was used to quantify the peripheral defocus of the retina and to observe the changes of peripheral defocus of the retina in patients treated with OK lens to evaluate its effect on the progression of myopia.METHODS:A cross-sectional study was used.To collect the children and adolescents with ametropia who were treated in the ophthalmology clinic of our hospital and reexamined regularly.75 patients(75 eyes)wearing OK lens were selected as OK group,and 80 children and adolescents(80 eyes)matched with diopter,sex and age of OK group were selected as non-OK group.The values of sex,age,intraocular pressure(IOP),axial length(AL),mean keratometer data(Km),total refraction difference value(TRDV),refraction difference value of the 15°retina(RDV-15°),refraction difference value of the 30° retina(RDV-30°),refraction difference value of the 45° retina(RDV-45°),refraction difference value of peripheral retina(RDV-(53°-15°))were compared between the experimental group and the control group.Whether there are differences between refraction difference value of the superior retina(RDV-S),refraction difference value of the inferior retina(RDV-I),refraction difference value of the nasal side retina(RDV-N),and refraction difference value of the temporal side retina(RDV-T).Within the group comparison 1: according to the number of equivalent spherical lenses,the OK group was divided into two groups: low myopia group(0D~-3.00D)and moderate myopia group(3.00D~-6.00D).The sex,age,IOP,AL,Km,TRDV,RDV-15°,RDV-30°,RDV-45°,RDV-(53°-15°),RDV-S,RDV-I,RDV-N and RDV-T were compared between the two groups.Within the group comparison 2:According to the wearing time of OK lens,the OK lens group was divided into three groups: < 6 months group,6 months-12 months group and > 12 months group.Sex,age,IOP,AL,Km,TRDV,RDV-15 °,RDV-30°,RDV-45°,RDV-(53°-15°),RDV-S,RDV-I,RDV-N and RDV-T were compared among the three groups.The correlation between RDV-(53°-15°)and TRDV,RDV-S,RDV-I,RDV-N,RDV-T was analyzed.RESULTS:1.There was no significant difference in sex,age and AL between OK lens group and non-OK lens group,but there was statistical significance in IOP and Km.IOP and Km in OK lens group were lower than those in non-OK lens group.2.TRDV,RDV-15°,RDV-30°,RDV-45°,RDV-(53°-15°),RDV-S,RDV-I,RDV-N and RDV-T in OK group were significantly lower than those in non-OK group(P < 0.001).3.OK group was divided into low myopia group and moderate myopia group.After comparison between the two groups,it was found that there was only statistical significance in age,AL and RDV-I,but there was no statistical significance in other aspects.The age and AL of moderate myopia group were higher than those of low myopia group,while the RDV-I of moderate myopia group was lower than that of low myopia group.4.The OK group was divided into three groups: < 6 months group,6 months to12 months group and > 12 months group.There were significant differences in TRDV,RDV-30°,RDV-45°,RDV-(53°-15°)and RDV-I among the three groups.After further pairwise comparison,it was found that among the indexes of TRDV,RDV-30°,RDV-45°,RDV-(53°-15°)and RDV-I,there was statistical significance between 6-12 months group and < 6 months group and > 12 months group,but there was no statistical significance between < 6 months group and >12 months group.TRDV,RDV-30°,RDV-45°,RDV-(53°-15°)and RDV-I in6-12 month group were lower than those in < 6 month group and > 12 month group.5.In the OK lens group,the Spearman correlation analysis between RDV-(53°-15°)and related factors showed that there was no correlation between RDV-(53°-15°)and RDV-N.There was a strong positive correlation between RDV-(53°-15°)and TRDV(r = 0.947,P < 0.001),and a general positive correlation between RDV-(53°-15°)and RDV-S,RDV-I,RDV-T(r =0.335,P < 0.01;r = 0.423,P < 0.001,P < 0.01).CONCLUSIONS:1.Through the detection of MRT,it was proved that the patients treated with OK lens presented the status of peripheral retinal myopia defocus.2.In this clinical study,there was no difference in peripheral retinal myopia defocus between low and moderate myopia treated by OK lens.3.OK lens showed more myopic defocus around the retina from 6 months to 12 months. |
