Background and objective:Spinal meningioma is one of the most common primary spinal tumors, second only to schwannoma in incidence, and patients often present with sensory and motor disorders.Meningiomas grow slowly and are mostly benign.Only about 4% of patients present with non-benign lesions and spinal meningiomas are usually not fatal and can be cured by surgery.Because of the benign characteristics, people often pay little attention to the prognosis of meningiomas, especially the long-term prognosis. The purpose of this study was to clarify the epidemiological and clinical characteristics of spinal meningiomas, analyze the factors related to postoperative neurological function recovery and other indicators,so as to deepen the understanding of meningioma, and provide the basis for the diagnosis, treatment of the patients with spinal meningiomas, and provide reference for the research on spinal meningioma in the future. Methods:Patients with spinal meningioma admitted to the Department of Neurosurgery,The First Hospital of Jilin University from January 2013 to November 2021 were collected, and the basic information, past medical history, personal history, operation data and pathologic data of the patients were descriptively analyzed.Patients were grouped according to the different clinical features and tumor-related characteristics, distribution characteristics of different groups were compared.Follow-up information of the patients was collected to inquire about the survival status and neurological function recovery of the patients, and multivariate logistic regression analysis was performed to find out the factors related to the improvement of postoperative neurological function. Results:Among the 93 patients with meningioma included in this study, the average age was 57.1 years, and the median follow-up time was 50.5 months. Most tumors occurred in middle-aged and elderly patients(87.1%),and the ratio of female patients to male patients was 8.3:1.There were 84 patients with WHO grade I tumors and 9 with WHO grade II tumors.A total of 2 recurrence cases and 9 deaths were found, all of which were caused by other diseases. In female patients,The diameter and volume of tumors are greater than male, the Simpson grades were related with the location of tumor and the existence of psammomma bodies, people with sensory disorder before operation bearing a higher improvement rate, while the existence of psammomma bodies herald a lower improvement rate,The improvement rates of patients with ventral-growing tumors were lower than that in patients with lateral-growing tumors. Conclusions:Spinal meningiomas are typically benign lesions, and are common in middle-aged and elderly patients. In females patients, the incidence of spinal meningioma is higher and tumors may have faster growth rate.Intratumor psammonma bodies formation and ventral location may be associated with a lower rate of improvement. The prognosis of spinal meningiomas is good and surgical treatment is the first choice of treatment.
