Objective:Aatypical meningioma(AM)is a rare tumor in the CNS,which is between benign and malignant meningioma. Its incidence has been increasing in recent years, but also because of the imaging findings and laboratory results have no specificity, and benign meningioma is extremely easy to confuse, preoperative diagnosis is often difficult, and because of its aggressive strong, difficult to make effective treatment postoperatively, through the analysis of the above characteristics of AM,so as to improve clinical understanding of the diagnosis and treatment and prognosis of the disease,To provide reference for the related research. Methods:Retrospective analysis of the 41 patients with pathologically confirmed AM admitted to our hospital during the period from September2016 to September 2020 at the Department of Neurosurgery, the First Hospital of Jilin University. The patient’s age, gender, duration, clinical data, performance,KPS scores, auxiliary examinations, treatment data, pathology data, and patient prognosis data were collected. And systematic analysis of them. Results:Among the 41 patients, there were 18 males and 23 females. The male to female ratio was 0.78:1;aged 43-72 years. The median age is 57,and the average age is 60.3,14 cases were older than 60 years, and 27 cases were below 60 years old(including 60 years old). Most of the clinical manifestations are symptoms of increased intracranial pressure as the main symptoms. The preoperative KPS score was ≥70 in 28 patients, and KPS score was<70 in 13 patients.Through head scans and enhanced MRI.12 cases were located at the skull base,29 cases were not located at the skull base,5 cases were invaded by tumor and 36 cases were not invaded by tumor.Statistical analysis showed that the survival time was longer in patients with the size of tumor、no invading brain tissue、GTR、Ki67 < 10% and postoperative radiotherapy and chemotherapy after surgery.Conclusion:1.The prognosis of AM patients was not related to the age and gender of the patients, the length of onset, whether the tumor was located in the skull base, preoperative KPS score and other factors.2.The prognosis of AM patients was related to the size of the tumor、STR、no invading brain tissue 、 Ki67 < 10% and no recurrence 。 3.Surgical resection is an independent factor affecting the recurrence of AM,and a high degree of resection is associated with a low recurrence probability.Adjuvant treatment after surgery can significantly reduce the recurrence rate and prolong the survival time of patients.
