Objective: To observe the clinical characteristics of patients with primary epithelial ovarian cancer and discuss the related risk factors that affect their prognosis.Methods: The clinical data of 282 cases with epithelial ovarian cancer who were diagnosed and treated in Suzhou Hospital of Nanjing Medical University for the first time from January 2007 to December 2018 were retrospectively analyzed.Univariate and multivariate Cox regression analysis were used to evaluate the various data affecting the overall survival of the disease.The receiver operating characteristic curve was used to evaluate the predictive power of the selected risk factors for survival at 1 and 3 years after surgery.Results: Among patients with epithelial ovarian cancer,postmenopausal women accounted for 53.9%(152/282).In FIGO stage,there were 118 patients(41.8%)in stage Ⅰ and Ⅱ,and 164 patients(58.2%)in stage Ⅲ and Ⅳ.The pathological classification was 24 cases(8.5%)with high differentiation,56 cases(19.9%)with moderate differentiation,and 202 cases(71.6%)with poor differentiation.After surgery,180 patients(63.8%)given consent to platinum and paclitaxel-based chemotherapy regimens,while 102 patients(36.2%)did not receive chemotherapy.Among the 180 patients who had received chemotherapy,106(58.9%)developed chemotherapy resistance,and 74(41.1%)did not develop chemotherapy resistance.Univariate analysis showed that menopause,history of tubal ligation,ascites,lymph node metastasis,FIGO stage,differentiation stage,chemotherapy resistance are all major determinants of survival(all P<0.05).Multivariate analysis showed that FIGO stage and drug resistance are independent factors which can determine the prognosis of EOC(HR=2.484,95%CI:1.807-3.415;HR=3.712,95%CI:1.953-7.058).ROC curve estimates the 1-year survival rate: AUC=0.779 95%CI: 0.717-0.841;3-year survival rate: AUC=0.661 95%CI: 0.592-0.731.Conclusion: FIGO stage clearly affects patient survival,and emphasis needs to be placed on early screening and prevention of ovarian cancer.Chemotherapy resistance,as a major problem affecting treatment,is closely related to negative prognosis,so it is urgent to discover and control the factors associated with drug resistance to prolong the overall survival time of patients.
