| Objective: Through the statistical investigation of patients with acute cerebral infarction, based on the DSA diagnosis results of patients with acute cerebral infarction,to further understand the relationship between CISS classification and cerebrovascular disease,so as to explore the relationship between CISS etiology classification and TCM syndrome type of acute cerebral infarction The relevance of TCM adds new content to the microcosmic treatment of TCM syndrome differentiation. Methods: From January 2019 to December 2020,111 patients who were diagnosed with acute cerebral infarction and were diagnosed with DSA in the Department of Neurology of the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine were enrolled. The relevant past history, current medical history, neurological examination, and physical examination of the patients were enrolled. The results of imaging examinations and laboratory examinations, collecting data from the four TCM consultations of patients, classifying each patient with TCM syndrome differentiation, and performing CISS standard classification for each patient who underwent cerebrovascular angiography,so as to analyze patients with acute cerebral infarction Based on the correlation between the CISS classification of the DSA examination results and the TCM syndrome types, and to further understand the relationship between the various TCM syndrome types and the cerebrovascular disease shown in the DSA examination, statistical analysis was carried out using SPSS26.0 statistical software.Results:1.General situation: A total of 111 subjects were included in this study, with an average age of(63.32?1.74)years old.Among them,74 were men(66.7%),37 were women(33.3%);60 were smoking(54.0%),and 65 were drinking(58.5%).In the CISS classification, large atherosclerosis was the majority. There are 57 people(51.4%);the TCM syndrome is the most common meridian-wind phlegm blocking collateral syndrome,58 people(52.3%);among them 44 people with diabetes(39.6%);50 patients with hyperlipidemia People(45.0%);85 people with hypertension(76.5),the majority of hypertension patients with grade 3,57 people(51.4%);21 people with heart disease(18.9%);patients with infarct position In the anterior circulation,92 people(82.9%)were the most common;the internal carotid artery stenosis was the main result of DSA,44 people(39.6%).2.There are statistical differences in the distribution of high-risk factors of acute cerebral infarction in the CISS classification(P<0.05).Smoking,drinking,diabetes,hyperlipidemia,and hypertension are in patients with aortic atherosclerosis and perforating artery disease. The proportion is relatively large in the middle, and heart disease accounts for a relatively large proportion in cardiogenic stroke. However, there was no statistical difference in gender in each classification(P>0.05).3.There are statistical differences in the composition of the onset location of different CISS classifications(P<0.005).Atherosclerosis,cardiogenic stroke, other causes or unexplained types mostly occur in the anterior circulation at the onset location.Branch artery disease mostly occurs in the posterior circulation.4.In the analysis of the composition of stenotic vessels in the DSA results of different CISS classifications, large atherosclerosis is statistically significant in the distribution of different stenotic vessels(P<0.05),of which the distribution of large atherosclerosis accounts for The largest is the internal carotid artery, followed by the middle cerebral artery, posterior cerebral artery, anterior cerebral artery, vertebral artery, and the least common carotid artery and basilar artery. There was no statistical significance in the distribution of blood vessels in perforating artery disease, cardiogenic stroke, and other or unexplained types(P>0.05).5.In the analysis of the composition of the vascular stenosis rate in the DSA results of different CISS classifications, the aortic atherosclerosis is the most distributed in severe stenosis, followed by total occlusion, moderate stenosis, and mild stenosis.Perforating artery disease has the most distribution of severe stenosis, followed by moderate stenosis, and the least distribution of mild stenosis and total occlusion.According to the chi-square test, there are statistically significant differences in large atherosclerosis and perforating artery disease in the CISS classification(P<0.05),and there is no statistical difference in other or unexplained types and cardiogenic stroke types(P>0.05).6.Comparative analysis of the high-risk factors of different TCM syndrome types of acute cerebral infarction. The analysis results show that the composition of hyperlipidemia, hypertension, diabetes, heart disease, and alcohol consumption rate of different TCM syndrome types is statistically different(P< 0.05).Among them, windphlegm blocking collaterals syndrome and Qi-deficiency and blood stasis syndrome are the most widely distributed, and there is no statistical difference in gender and smoking rate(P>0.05).7.Comparative analysis of the differences in the composition of the different TCM syndrome types. The analysis results show that the middle meridian is mainly distributed in the anterior circulation.Among them, the syndrome of apoplexy wind phlegm blocking collaterals and the syndrome of qi deficiency and blood stasis are mostly distributed, and the central organs are mainly distributed in the back Circulation, the distribution of phlegm-dampness syndrome and phlegm-fire internal closure syndrome are mostly distributed, and the composition of the onset position of different TCM syndrome types is statistically different(P<0.05).8.Comparative analysis of the differences in the composition of DSA stenosis vessels of different TCM syndromes. The analysis results show that the internal carotid artery stenosis is in the middle meridian wind phlegm blocking collaterals syndrome,qi deficiency and blood stasis syndrome, wind and fire upper disturbance syndrome, yin deficiency and wind movement In the visceral organs,phlegm-heat internal obturation and stenosis of the blood vessels accounted for the most vertebral artery and posterior cerebral artery, and the visceral phlegm-dampness syndrome accounted for the basilar artery. The vascular composition of DSA stenosis with different TCM syndromes was statistically different(P<0.05).9.In the distribution of vascular stenosis rate in various TCM syndromes,P<0.001 after Chi-square test, the analysis of the difference in the composition of intracranial and extracranial vascular stenosis of different TCM syndromes is statistically significant, most of which are moderate stenosis and severe Predominantly narrow.10.Comparative analysis of the differences in the composition of TCM syndromes of different CISS types. After Chi-square test, the analysis of the differences in composition of TCM syndromes of different CISS types was statistically significant(P<0.05).11.Using the Spearman correlation analysis method, the CISS classification is related to the analysis of traditional Chinese medicine;the patient’s smoking, drinking rate, combined diabetes, hypertension, hyperlipidemia and heart disease, the location of the disease, the narrowed blood vessel, and the degree of blood vessel stenosis are all related There is a correlation with CISS classification and TCM syndrome type, gender and age have no correlation with CISS classification and TCM syndrome type. Conclusion: The CISS classification of acute cerebral infarction based on DSA is correlated with TCM syndrome.1)Except for perforating artery disease, which mostly occurs in the posterior circulation, the other subtypes are mostly distributed in the anterior circulation. The internal carotid artery is the site of vascular stenosis in large atherosclerosis, and most of it is severe stenosis.2)The middle meridian is mainly distributed in the anterior circulation.Among them, the syndrome of apoplexy wind phlegm blocking the collaterals and the syndrome of qi deficiency and blood stasis are mostly distributed, mostly involving the internal carotid artery, and most of them are moderate to severe stenosis. The viscera are mainly distributed in the posterior circulation, mostly involving the vertebral artery and basilar artery, and the degree of vascular stenosis is mostly severe or occluded.3)In the CISS classification of acute cerebral infarction, among the TCM syndromes, aortic atherosclerosis, perforating artery disease, unexplained and other causes have the most syndromes of wind phlegm blocking the collaterals in the middle meridian, and deficiency of qi and blood stasis in cardiogenic stroke. The most certification. |
