The Clinical Efficacy Observation On Shenzhe Zhenqi Decoction And Litan Decoction In Treating AECOPD With Type Ⅱ Respiratory Failure

ObjectiveTo observe the clinical efficacy of Shenzhenqi Decoction and Litan decoc tion in the treatment of AECOPD complicated with type II respiratory failure,and optimize the clinical treatment scheme of AECOPD complicated with type II respiratory failure by Traditional Chinese Medicine.MethodsSixty patients who met the requirements of this study and were diagnos ed with exacerbation of chronic obstructive pulmonary disease（COPD）compli cated with type II respiratory failure in wstern medicine and syndrome differe ntiation of“lung and kidney Qi deficiency,phlegm and turbidities obstructing lung”in TCM,were randomly divided into treatment group and control group,with 30 cases in each group.Both groups received conventional western medi cine treatment,such as oxygen therapy,spasmolysis and asthma relief,anti-infecti on,and non-invasive ventilator assisted ventilation.On this basis,the treatment gr oup was given“Shenzhe Zhenqi and Litan Decoction”for 12 days.The differe nces in clinical efficacy between the two groups before and after treatment w ere compared,and evaluate the clinical efficacy and safety.Results1.Baseline comparison and Safety evaluation:Baseline data were comparabl e and the safety was good.2.Evaluation of the clinical total efficiency:The total efficiency in therape utic group is 96.67%that higher than that in control group（83.33%）,there was significant difference between the two groups（P<0.01）.3.Comparison of total scores of TCM syndromes:The total scores of TC M syndromes in the two groups were obviously improved,and the improveme nt was more obvious in the treatment group（P<0.01）.4.Evaluation of single TCM symptom score:The single TCM symptom sc ore in the two groups were obviously improved（P<0.01）,The improvement of scores of cough,sputum,shortness of breath,wheezing,chest distress and fatig ue of treatment group was better（P<0.05）.There was no statistical significance in the scores of abdominal distension,spontaneous perspiration,lumbar acid an d tinnitus after treatment（P>0.05）.5.m MRC score and CAT score:The m MRC score and CAT score in the two groups showed improvement,and the treatment group was more obvious（P<0.05）.6.Evaluation of inflammatory indicators:WBC,NEUT,NEUT%and CRP in dexes in the two groups showed improvement,and the treatment group was m ore obvious（P<0.05）.7.Evaluation of blood gas analysis indexes:Both groups could increase Pa O₂,Sp O₂,Pa O₂/Fi O₂,and decrease Pa CO₂,but the treatment group was better than the control group（P<0.05）.ConclusionThe clinical efficacy of Shenzhenqi decoction and Litan decoction combin ed with conventional western medicine therapy in the treatment of AECOPD combined type II respiratory failure is superior to that of conventional western medicine treatment alone.It can effectively control clinical symptoms,improve blood gas analysis indexes and inflammatory indexes,and the safety was good during the treatment.
