| ObjectiveThe purpose of this study was to explore the related factors affecting the prognosis of patients with hepatitis B related chronic liver failure and its correlation with TCM syndrome types.MethodThe clinical data of 175 patients with hepatitis B-related chronic liver failure who were hospitalized in the Department of infection,Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from July 2016 to December 2020 were collected by retrospective analysis according to the standards of 2018 guidelines for diagnosis and treatment of liver failure,including age,gender,whether to take antiviral drugs before admission,laboratory indicators after admission,complications,complications,complications,complications Antiviral treatment,TCM syndrome types,TCM treatment and prognosis,etc.According to the outcome,175 patients were divided into improved group and unhealed group,and their clinical characteristics and correlation with TCM syndrome types were analyzed.Results1.Of the 175 patients included in this study,101 were in the improvement group,74 were in the non recovery group(including 35 deaths).2.There was no significant difference in age,gender,whether there was antiviral therapy before admission,HBV-DNA,globulin,ascites,hepatic encephalopathy,whether there was liver cancer,antiviral therapy after admission and traditional Chinese medicine treatment between the two groups(P > 0.05).3.There were significant differences in WBC,neutrophil percentage,neutrophil /lymphocyte ratio,platelet,C-reactive protein,alanine aminotransferase,aspartate aminotransferase,glutamyltransferase,cholinesterase,total bilirubin,albumin,sodium,international standard ratio,prothrombin activity,hepatorenal syndrome,gastrointestinal bleeding,spontaneous peritonitis between the two groups There were significant differences in electrolyte disorder,MELD-Na score,MELD-Na score and CTP score(P < 0.05);4.there were differences in TCM syndrome types between the two groups,the difference was statistically significant(P < 0.05),the improvement rate of the syndrome of stasis and heat accumulation was significantly higher than that of yin deficiency and blood stasis and toxin syndrome.5.Binary logistic regression analysis showed that hepatorenal syndrome,gastrointestinal bleeding,glutamyltransferase,MELD Na score were independent risk factors for the prognosis of patients with HBV related chronic liver failure,albumin and prothrombin activity were protective factors for patients with HBV related chronic liver failure.6.Binary logistic regression analysis showed that gastrointestinal bleeding and meld Na score were independent risk factors for the prognosis of patients with chronic liver failure of damp heat toxin accumulation syndrome;neutrophil percentage and meld Na score were independent risk factors for the prognosis of patients with chronic liver failure of yin deficiency and Blood Stasis Toxin syndrome;meld Na score was independent risk factor for the prognosis of patients with chronic liver failure of Yang deficiency and Blood Stasis Toxin syndrome There was no significant influence factor on the prognosis of patients with chronic liver failure.Conclusion1.Hepatorenal syndrome,gastrointestinal hemorrhage,gamma-glutamyl transferase and MELD-Na scores are independent risk factors for the prognosis of patients with hepatitis B related chronic liver failure.Albumin and prothrombin activity are the protective factors of patients with hepatitis B related chronic liver failure.2.MELD-Na score can predict the prognosis of patients with hepatitis B related chronic liver failure.3.The prognosis of different TCM syndrome types of hepatitis B related chronic liver failure is different,and the influencing factors of prognosis of different TCM syndrome types of hepatitis B related chronic liver failure are different,which provides a new idea for the prognosis prediction of chronic liver failure in clinic. |
