ObjectiveThis subject intends to observe the clinical efficacy of Jiujiecha gargle combined with Yiweitang granules in the prevention and treatment of radiation oral mucositis through randomized controlled trials,and explore new ideas and new methods for prevention and treatment of radiation oral mucositis.The application of radiotherapy tolerance and quality of life in cancer patients provides clinical evidence.MethodsA total of 60 patients with head and neck tumors requiring radiotherapy who were admitted to our hospital from June 2020 to June 2021 were selected and randomly divided into observation group and control group,with 30 cases in each group.Both groups were given intensity-modulated radiotherapy.The control group was given regular oral care and other symptomatic treatment during radiotherapy according to the Expert Consensus on Radiation Oral Mucositis Prevention and Treatment Strategies(2019).The treatment group was given symptomatic treatment such as routine oral care in the control group.In addition,Jiujiecha gargle and Yiweitang granules were taken orally to observe the incidence of severe radiotherapy induced oral mucositis(RTOM),the occurrence time of various levels of RTOM,TCM symptom score change,pain numerical rating scale(NRS)score change,Karnofsky(KPS)score change,and epidermal growth factor(EGF)changes in saliva,objective evaluation of Jiujiecha gargle combination The clinical efficacy of Yiweitang granules in preventing and treating radiation oral mucositis.Results(1)The samples from different groups showed consistency in the seven general data of gender,age group,smoking,tumor type,and clinical stage,chemotherapy,PS score before treatment,and there was no difference(P>0.05).(2)In terms of the incidence of RTOM Ⅲ and Ⅳ,the treatment group was lower than the control group,but the difference was not statistically significant(P>0.05);In terms of the occurrence time of RTOM,the occurrence time of the treatment group was later than that of the control group(P <0.05).(3)The inter-subject effect of TCM symptom scores before and after treatment in the two groups was statistically significant(P <0.05).And the interaction between time point and group was significant(P <0.05).The LSD-t pairwise comparison test results after the event showed that after 3 weeks of treatment,the total scores of TCM symptoms of the two groups of patients were significantly higher than that before treatment,and the difference was statistically significant(P <0.05);TCM after 6 weeks of treatment The symptom score was significantly higher than the score after 3 weeks of treatment(P <0.05).The TCM symptom scores of the two groups of patients were compared before treatment and at 3weeks of treatment,and there was no significant difference(P>0.05).After 6 weeks of treatment,both groups were higher than before treatment,but the increase in the control group was more significant.The group difference was statistically significant(P <0.05).(4)The NRS scores of the two groups of patients before and after treatment are based on time point and group as the source of the inter-subject effect,which is statistically significant(P <0.05).And the interaction between time point and group was significant(P<0.05).After 3 weeks and 6 weeks of treatment,the NRS scores of the two groups were significantly higher than those before treatment,and the difference was statistically significant(P<0.05);the NRS score of 6 weeks of treatment was significantly higher than that of 3 weeks of treatment(P<0.05)).The NRS scores of the two groups were compared before treatment,and the difference was not statistically significant(P>0.05).After treatment for 3 weeks and 6 weeks after treatment,both groups were higher than before treatment,but the increase in the control group was more significant.The difference between the two groups It is statistically significant(P <0.05).(5)The inter-subject effect of the KPS scores of the two groups of patients before and after treatment based on time points was statistically significant(P <0.05).There was no significant difference in the KPS scores of the two groups before treatment(P>0.05);after3 weeks and 6 weeks of treatment,both groups decreased compared with before treatment,but the decrease in the control group was more significant,and the difference between the two groups was statistically significant(P <0.05).(6)The test results of the changes in EGF levels of the two groups of patients before and after treatment showed that the EGF levels of the two groups of patients were significantly lower than before the treatment(P <0.05).But after treatment,the EGF level of patients in the treatment group was higher than that in the control group(P <0.05).(7)Age,gender,smoking,tumor type,EGF level before treatment,TCM symptom score before treatment,chemotherapy,and PS score before treatment are not independent factors that affect patients with grade III and above RTOM(P>0.05).ConclusionJiujiecha gargle combined with Yiweitang granules has a good clinical effect in the prevention and treatment of radiation oral mucositis.Compared with simple conventional oral care,its main advantages are: 1.delay the occurrence of RTOM;2.Decrease the total score of TCM symptoms,relieve the symptoms and signs of patients.3.Reduce the patient’s NRS score,relieve pain,and improve the quality of life of the patient.4.Relieve the decrease in KPS score caused by radiotherapy and maintain the patient’s good functional status during radiotherapy;5.Inhibit the decrease of EGF level in saliva,and accelerate the proliferation and regeneration of oral mucosal epithelial cells in patients.
