Construction Of Clinical Nursing Pathway For Day Surgery Of Hip Replacement Based On Omaha System

ObjectiveWith the international standardized nursing language Omaha system as the framework,based on literature research and case review,the concept of rapid rehabilitation and traditional Chinese medicine nursing technology are integrated,and the Delphi method is used to construct a clinical pathway oriented to the core nursing problems of patients with hip replacement day surgery.The nursing path provides reference and data support for the formulation and standardization of the clinical nursing path of hip replacement day surgery.MethodThrough literature research,we extracted high-frequency words and literature views related to clinical nursing pathway of hip replacement day surgery.Collect the medical records of patients undergoing hip arthroplasty under the concept of rapid rehabilitation in the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine from July 2019 to July 2020 for case retrospective analysis.On this basis,the questionnaire of clinical nursing pathway was formed.Through three rounds of Delphi method,combined with the inclusion criteria of item score(mean≥3.5 and coefficient of variation<0.25)and expert opinions,the content of clinical nursing path of hip replacement day surgery was finally determined.ResultThrough literature research and case retrospective analysis,we preliminarily determined the content of the questionnaire for the clinical nursing path of hip replacement day surgery,which includes two parts: "core nursing issues" and "clinical nursing path".Three rounds of Delphi expert letter inquiries were conducted for 16 experts,and the experts’ active coefficients were all 100%,and the authority coefficients were 0.861,0.855,and 0.847,respectively.That authority coefficient of the experts is comparatively high.After sorting out and analyzing the expert’s scoring results and opinions,the questionnaire was revised.In the end,the coordination coefficient of "core nursing problems" is 0.401,the coefficient of variation is between 0.000～0.190,and the Cronbach coefficient is 0.892;the coordination coefficient of "Clinical Care Path" is 0.396,and the coefficient of variation fluctuates between 0.000～0.228,Cronbach The coefficient is 0.986,which shows that the questionnaire has good expert consistency and reliability,and the results are stable and reliable.As a result,32 core nursing problems have been identified,and a clinical nursing path of hip replacement day surgery with a total of 181 nursing operation items in 4 stages has been formed.ConclusionBy using the methods of literature research,case review and Delphi,the clinical nursing pathway of hip replacement day operation was constructed.Objective to verify the applicability of traditional Chinese medicine nursing concept and technology in day surgery for total hip athroplasty,so as to provide reference for the application of traditional Chinese medicine nursing in the daily nursing of daytime operation.Through the use of the Delphi method,it proves its scientificity and practicality,and promotes its application and promotion in the construction of day surgery nursing pathways.By constructing the clinical nursing path of hip replacement day surgery,standardizing nursing work,improving the quality of nursing,and promoting the rehabilitation of patients,it lays the foundation for its clinical practice.
