Objective:To investigate the changes of blood lipid and lung function aft er bronchoscopic treatment of endobronchial tuberculosis.Methods:100 patients with bronchial tuberculosis diagnosed by bronchoscopy in our hospital from January 2013 to December 2019 were collected.Among them,50 cases in the control group received conventional anti-tuberculosis drug treatment,and 50 cases in the observation group based on the control group combined with the local injection treatment under bronchoscope.After 3 months of treatment,the two groups of patients were compared in terms of treatment effectiveness,time for remission,time for the sputum bacteria to become negative,and changes in blood lipids,eosinophils and lung function levels.Result:1.Demographic characteristics of the two groups:the observation group had 17 males,33 females;the control group had 22 males,28females.There was no statistically significance in the gender difference between the two groups(P>0.05).The age of the observation group(52.20±7.62 years),and the age of the control group(49.48±8.10 years).There was no statistically significance in the age difference(P>0.05).2.The effective rate of treatment in the two groups:the observation group was 92.00%,while the control group was lower than the observation group,only 74.00%.The difference was statistically significant(P<0.05).3.Comparison of symptom relief and sputum negative time:the time of symptom relief in the observation group was(19.42±4.85 days),while in the control group was(37.65±9.41 days).The difference between the two groups was statistically significant(P<0.05).The time for the sputum to become negative in the observation group was(61.39±12.27 days),while in the control group was(76.52±15.30 days).The difference between the two groups was statistically significant(P<0.05).4.Comparison of blood lipid levels4.1 Comparison of blood lipid levels between the two groups before treatment:the values of TC,TG,HDL-C and LDL-C in the observation group before treatment were:(4.67±0.98mmol/L),(1.34±0.30mmol/L),(1.66±0.40mmol)/L),(2.95±0.68mmol/L),while the control group were(4.34±0.85mmol/L),(1.25±0.27mmol/L),(1.52±0.32mmol/L),(3.23±0.75mmol/L).There was no statistically significance in the difference(P>0.05).4.2 Comparison of blood lipid levels between the two groups after treatment:the values of TC,TG,HDL-C and LDL-C in the observation group were(3.05±0.67mmol/L),(0.90±0.20mmol/L),(0.84±0.18mmol/L),(2.01±0.45mmol/L),which were significantly lower than the average levels of the control group(3.39±0.78mmol/L),(0.83±0.14mmol/L),(0.92±0.21mmol/L),(1.83±0.37mmol/L).The difference was statistically significant(P<0.05).4.3 Comparison of blood lipid levels between the two group before and after treatment:the values of TC,TG,HDL-C and LDL-C in the observation group after treatment were(3.05±0.67mmol/L),(0.90±0.20mmol/L),(0.84±0.18mmol/L),(2.01±0.45mmol/L),which were significantly lower than the average levels before treatment(4.67±0.98mmol/L),(1.34±0.30mmol/L),(1.66±0.40mmol/L),(2.95±0.68mmol/L).The difference was statistically significant(P<0.05).the values of TC,TG,HDL-C,and LDL-C in the control group after treatment were(3.39±0.78mmol/L),(0.83±0.14mmol/L),(0.92±0.21mmol/L),(1.83±0.37mmol/L),which were lower than the average levels before treatment(4.34±0.85mmol/L),(1.25±0.27mmol/L),(1.52±0.32mmol/L),(3.23±0.75mmol/L).The difference was statistically significant(P<0.05).5.Comparison of eosinophil levels between the two groups:the eosinophil values of the observation group before treatment were(0.92±0.18*10~9/L),and the values of the control group were(0.87±0.17*10~9/L).The difference between the two groups was not statistically significant(P>0.05).After treatment,the eosinophil value in the observation group was(0.85±0.17*10~9/L)and the value of the control group was(0.81±0.16*10~9/L).There was no statistically significance in the difference(P>0.05).The values of the observation group before and after treatment were(0.92±0.18*10~9/L)and(0.85±0.17*10~9/L).There was no statistically significance in the difference(P>0.05).The values of the control group before and after treatment were(0.87±0.17*10~9/L)and(0.81±0.16*10~9/L).There was no statistically significance in the difference(P>0.05).6.Comparison of lung function levels between the two groups6.1 Comparison of lung function levels between the two groups before treatment:the values of FVC,FEV1/FVC and PEF of the observation group were(2.41±0.54L),(67.83±7.54%),(2.87±0.66L/s),and the values of the control group were(2.25±0.49L),(69.02±7.67%),(2.61±0.59L/s).There was no statistically significance in the difference(P>0.05).6.2 Comparison of lung function levels between the two groups after treatment:FVC,FEV1/FVC,and PEF levels in the observation group were(3.52±0.83L),(79.56±8.84%),(4.07±0.97L/s),which were significantly higher than those in the control group(3.09±0.74L),(75.35±8.37%),(3.63±0.85L/s).The difference between the two groups was statistically significant(P<0.05).6.3 Comparison of lung function levels between the two group before and after treatment:FVC,FEV1/FVC,and PEF levels in the observation group after treatment were(3.52±0.83L),(79.56±8.84%),and(4.07±0.97L/s),which were significantly higher than the levels before treatment(2.41±0.54L),(67.83±7.54%),(2.87±0.66L/s).The difference before and after treatment was statistically significant(P<0.05).FVC,FEV1/FVC,and PEF levels in the control group after treatment were(3.09±0.74L),(75.35±8.37%),(3.63±0.85L/s),which were significantly higher than the levels before treatment(2.25±0.49L),(69.02±7.67%),(2.61±0.59L/s).The difference before and after treatment was statistically significant(P<0.05).7.Correlation analysis of blood lipids and lung function:FVC and the 4 items TG,TC,LDL-C and HDL-C were all significant.The correlation coefficient values were-0.393,-0.317,-0.283,-0.425,and the correlation coefficient values were all less than 0,which means the FVC had a negative correlation with TG,TC,LDL-C,and HDL-C.PEF and the 4 items TG,TC,LDL-C,and HDL-C were all significant.The correlation coefficient values were-0.302,-0.379,,-0.368,and-0.442,and the correlation coefficient values were all less than 0,which means that there was a negative correlation between PEF and TG,TC,LDL-C,and HDL-C.FEV1/FVC and the 4 items TG,TC,LDL-C,and HDL-C were all significant.The correlation coefficient values were-0.367,-0.280,-0.422,-0.455,and the correlation coefficient values were all less than 0,which means that FEV1/FVC had a negative correlation with TG,TC,LDL-C,and HDL-C.Conclusion:1.The levels of FVC,FEV1/FVC and PEF in patients with bronchial tuberculosis after anti-tuberculosis drugs and microscopic treatment increased more significantly than those of non-microscopic treatment patients.2.The blood lipid level of patients with bronchial tuberculosis decreased after systematic anti-tuberculosis treatment,but the blood lipid level of patients with bronchial tuberculosis decreased more significantly after 3 months of treatment under auxiliary bronchoscopy.3.Pulmonary function(FVC,PEFand FEV1/FVC)was negatively correlated with blood lipid levels(TG,TC,LDL-C,HDL-C)in pati ents with bronchial tuberculosis.
