Objective: To investigate the risk factors of lacunar infarction(LI)patients with Cerebral microbleeds(CMBs),and to analyze the threshold of fasting blood glucose(FPG)and glycated hemoglobin(Hb Alc)which predict LI patients occur CMBs.Accordingly,their treatment can be efficiently,and stroke,cognitive impairment and other cerebrovascular diseases can be as soon as possible.Methods: Patients with LI,who were admitted to the Neurology Department in our hospital from September,2018 to August 2020 were selected and divided into CMBs group(77 cases)and NCMBs group(170 cases)on the basis of SWI.The clinical information were recorded and edited.The data were analyzed by SPSS25.0.The counting data: chi-square test;the measurement data(normal distribution): t test for two groups,ANOVO for multiple groups;Rank data: Spearman correlation analysis;Multivariate Logistic regression analysis were used to explore independent risk factors of CMBs.The ROC curves determined the threshold of LI combined with CMBs for FPG and Hb Alc.Results: Univariate analysis of counting data showed that the differences in gender and alcohol consumption were not statistically significant between the two groups(P >0.05);the differences in smoking,hypertension,diabetes,and atrial fibrillation were statistically significant(P< 0.05).Univariate analysis of measurement data suggested that the differences in HDL-C were not statistically significant(P>0.05);the differences in age,FPG,Hb Alc,NLR,LDL-C,Cys C,UA,Hcy,Fazekas score and number of LI were statistically significant(P<0.05).Multivariate logistic regression analysis showed that the differences in age,hypertension,diabetes,AF,FPG,Hb Alc,LDL-C,Cys C,Hcy,Fazekas score and number of LI were statistically significant between the two groups(P<0.05).The blood glucose indexes were checked with ANOVO,and the results showed that with the degree of CMBs increased gradually,serum FPG and Hb Alc levels were remarkable increased(P<0.05).Spearman correlation analysis of blood pressure grading showed that the severity of CMBs was moderately positive correlated with blood pressure grading(P < 0.05,RS = 0.447).The ROC curves of FPG,Hb Alc showed that the area under the curve of FPG was 0.854,the sensitivity was 89.6%,the specificity was 77.1%,and the threshold was 8.23mmol/L.The area under the curve of Hb Alc was0.843,the sensitivity was 75.3%,the specificity was 81.2%,and the threshold was6.85%.Conclusions: Advanced age,hypertension,diabetes,AF,high level of FPG,Hb Alc,LDL-C,Cys C,Hcy,and WMH are primary risk factors leading to the formation of CMBs in LI patients;the higher levels of FPG,Hb Alc and blood pressure grade,the more the number of CMBs;when FPG≥8.23 mmol/L or Hb Alc≥6.85%,the diagnostic efficacy of predicting the formation of CMBs is best in LI patients.
