Objective:To explore the correlation between prognostic nutritional index and clinical characteristics(such as gender,age,smoking history,chest pain,chest tightness,blood in sputum,weight loss,clinical stage,site,history of chemotherapy,pathological type)and side effects of radiotherapy in patients with advanced non-small cell lung cancer(NSCLC).Methods:Using retrospective analysis,One hundred and three cases of locally advanced non-small cell lung cancer patients of this study were selected from radiotherapy medicine ward of people’s hospital in Liaocheng city from January 1,2012 to December 1,2018,who were successfully diagnosed by pathological for the first time.According to the inclusion criteria and exclusion criteria,finally 82 patients with NSCLC were enrolled in this experiment.Clinical data for evaluating the nutritional status were collected from the electronic medical records of selected patients,including serum albumin level and lymphocyte count before treatment,and radiotherapy side effects,such as fatigue,nausea,fever,cough,eating pain,bone marrow suppressionand.And the PNI was calculated as10*serum album-in value(g/L)+5*total lymphocyte count(~*10^9/L)in peripheral blood.Statistical software SPSS17.0 was used to explore the relationship between PNI and potential factors such as age,gender,tissue type,site,history of chemotherapy,and the incidence of adverse reactions,and to analyze the correlation with PNI.The continuity of the variables are indicated by mean plus standard deviation,t test is adopted for the comparison between the means,and counting data are tested by chi-square test.P<0.05 was considered statistically significant.Results:1.The median age of 82 patients was 65 years old,including 62 males and 20 females,the sex ratio was 2.82:1,smoking is 58 cases(70%),52 cases(63.41%)of squamous cell carcinomas,22 cases(26.83%)of adenocarcinoma,and other types of 8 cases(9.76%),37cases(45.12%)were located in the left lung,45 cases(54.88%)in the right lung,64 patients with stage Ⅲ(78.05%),while the percentage of Ⅳstage patients was 18 cases(21.95%),35(42.68%)cases who received chemotherapy,blood in the sputum was 34cases(41.46%),the chest pain was 24 cases(29.27%),There were 39 cases of chest distress(47.56%),the weight loss patients were 13 cases(15.85%).2.PNI was significantly correlated with patients’ age and history of chemotherapy(P<0.05),while the results states that PNI has no obvious correlation to the patient’s smoking,gender,blood in sputum,chest pain,chest tightness,weight loss,clinical stage,primary tumor site and histological type,and the differences of them were no statistically significant(P>0.05).3.There was no correlation between the clinical data and the incidence of radiotherapy complications.4.The patients were divided into the low PNI group and the high PNI group with the mean value of 43.65 as the boundary value.The results showed that the incidence of bone marrow suppression,radiation pneumonia,nausea,fever and fatigue during radiotherapy in the high PNI group was lower than that in the low PNI group,and the difference was statistically significant.The incidence of radioactive esophagitis in the high PNI group was lower than that in the low PNI group,but thedifferences of them were no statistically significant(P>0.05).Conclusions:PNI can reflect the nutritional status of patients,and the detection of this index is simple,rapid,cheap and widely used in clinical practice.The low prognostic nutritional index indicates the high incidence of side reactions in the radiotherapy of patients with middle and advanced non-small cell lung cancer,which can be used for nutritional supplement and guiding treatment and has good clinical application value.
