| Objective:Proximal humeral fracture is one of the most common types of multiple fractures in clinic.It is common in the elderly,and its incidence is increasing.Surgical treatment is a commonly used method at present.Locking plate is a material commonly used for internal fixation of fracture in recent years.It has the advantages of less trauma and good fixation effect,which is conducive to the recovery of joint function in patients after operation.The purpose of this study was to investigate the effects of locking plate on surgical trauma,pain and joint function recovery in patients with proximal humerus fracture through observation and analysis of patients with proximal humerus fracture.Methods:106 cases of proximal humeral fracture were selected from January 2017 to February2018.According to the different surgical methods,the patients were divided into control group(53 cases)and observation group(53 cases).The patients in control group received conventional open reduction and internal fixation,while the patients in observation group were treated with locking plate.Results:(1)There was no significant difference in the average age,fracture type,Neer fracture type and the causes of fracture between the control group and the observation group(P>0.05).(2)The clinical total effective rate of the observation group was higher than that of the control group,with statistical significance(P<0.05).(3)The operation time,intraoperative bleeding volume,intraoperative fluoroscopy time,incision length and postoperative drainage of patients in the observation group were better than those in the control group,with statistical significance(P<0.05).(4)The braking duration,hospitalization time and wound healing time of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).(5)The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).(6)The total satisfaction of patients in the observation group was higher than that in the control group,with statistical significance(P<0.05).(7)The excellent rate of the observation group was higher than that of the control group,with statistical significance(P<0.05).(8)The scores of shoulder pain,function,range of motion and anatomical position in the observation group were higher than those in the control group,with statistical significance(P<0.05).(9)The VAS pain score of the observation group was lower than that of the control group on the 1st day,7th day and 1st month postoperatively(P<0.05).(10)Six months after operation,the scores of psychological function,social function,material life and physical function of the observation group were higher than those of the control group,with statistical significance(P<0.05).Conclusions:1.Locking plate can effectively reduce the operation time,intraoperative bleeding volume,intraoperative fluoroscopy time and incision length of patients with high proximal humerus fracture,reduce the occurrence of complications and improve satisfaction.2.Locking plate can alleviate the pain of patients with proximal humeral fracture after operation and improve the quality of life of patients.3.Locking plate can promote the recovery of joint function in patients with proximal humeral fracture after operation.Significance:Proximal humeral fracture is a common injury disease in orthopaedics,accounting for about 4% and 5% of all fractures.The incidence of proximal humeral fracture is increasing year by year.Locking plate has the advantages of less trauma,better fixation effect and less risk of screw loosening.It has less damage to soft tissue and blood supply.It is safe and effective in the treatment of proximal humeral fractures.However,the reports are inconsistent.This study is to observe the effect of locking plate in the treatment of proximal humerus fracture,and to provide guidance for clinical treatment of proximal humerus fracture with locking plate. |
