| ObjectiveIn this study,the ancient and modern medical record cloud platform was used to conduct data mining on the data of mentor Professor Liu Ling in the treatment of patients with hypnotic drug-dependent sleep disorder,to summarize the basic characteristics of the hypnotic drug-dependent sleep disorder population and the mentor’s experience in syndrome differentiation,so as to better apply it to clinical practice.MethodsA total of 46 patients diagnosed and treated as hypnotic drug-dependent sleep disorder in the Expert Clinic of Professor Liu Ling,Encephalopathy Department,Hubei Provincial Hospital of Traditional Chinese Medicine from November 2020 to November2021 were included in the study.After data entry and data standardization,data mining was performed on general information,diagnosis and treatment information,and syndrome differentiation information using the ancient and modern medical record cloud platform statistical analysis,correlation analysis,and complex network analysis functions,and curative effect analysis was performed using SPSS 27.0.Results1.Basic case informationThere were 10 males and 36 females aged from 26 to 72 years old with an average age of 56 years old.89.13% of them were married;Type of work for the worker accounted for the highest,44%;The patients were mainly assigned to day shift（97.83%）.The most patients had intermediate educational level（junior high school,senior high school and junior high school）,accounting for 60.87%.The proportion of patients living with loved ones was higher for 58.70%.2.Related information on diagnosis and treatment of hypnotic drug dependent sleep disorderThe main symptoms of patients with hypnotic drug-dependent sleep disorder were difficulty in falling asleep and easy to wake up at night.Mental factors were the main definite precipitating factors,but most patients did not complain about the obvious ones.63% of patients had a previous illness;The results of the Pittsburgh Sleep Quality Index（PSQI）showed that 58.70% of patients had poor sleep quality,while the results of the Insomnia Severity Index（ISI）showed that 73.92%of patients had moderate insomnia.The results of Hamilton Anxiety Scale（HAMA）showed that about two-thirds of the patients were accompanied by anxiety,while the results of Hamilton Depression Scale（HAMD）showed that about one-quarter of the patients were accompanied by depression.3.The information of TCM syndrome differentiation and tre AtmentThe main syndrome types of hypnotic drug-dependent sleep disorder were deficiency of heart and spleen,yin deficiency and exuberant fire,and liver stagnation and fire transformation syndrome.The main pathogenic factors were qi stagnation,qi deficiency and blood deficiency.The location of the disease was mainly in the heart,spleen and liver.For tongue texture,red,pale red and pale tongue are more common.The most common tongue coating is thick and white.The pulse manifestations were string and thin.The principal treatment is tranquilization.Patients were treated with more traditional Chinese medicine dialectical prescriptions（69.57%）,and each patient was given sleep health education.The most commonly used prescriptions of tutor were Suanzaoren Decoction,Chaihu Shugan Powder,Anmei Dan and Guipi Decoction.The most frequently-used Chinese herbs were Ziziphi Spinosae Semen,Poria,Rhizoma Ligustici Chuanxiong,Fructus Schisandrae Chinensis,Rhizoma Acori Tatarinowii,and Radix Salviae Miltiorrhizae.Among the four drugs,the neutral ones were used most frequently,followed by warm and slight cold ones.The sweet ones were the most used in the five flavors,followed by the bitter and pungent ones.Meridian tropism refers to the meridians of heart,liver,and spleen.The pair with the highest correlation degree of TCM was Radix Salviae Miltiorrhizae and Semen Ziziphi Spinosae.The core drugs were Semen Ziziphi Spinosae,Radix Salviae Miltiorrhizae,Poria,Fructus Schisandrae Chinensis,and Rhizoma Ligustici Chuanxiong.4.Efficacy analysisThe reducing rate of PSQI score scale showed that the total effective rate（cured+markedly effective+effective）was80.43%.The hypnotic drug reduction rate showed that the total effective rate（clinical control+effective）was 82.61%;The total scores of PSQI scale,ISI scale and hypnotic drug consumption before and after treatment were significantly different（P<0.01）.Conclusion1.The majority of patients with hypnotic drug-dependent sleep disorder were women,and the majority of them were middle-aged or elderly.The proportion of patients who were married and lived with their loved ones was higher;The highest proportion of workers in the type of work;Most of them have intermediate education（junior high school,senior high school and junior high school）.As the work schedule of included cases was almost day-shift,the results showed that the work schedule had little impact on the patients.It can be deduced that the menopausal women,those with more trivial matters in life,greater work pressure and intermediate educational level are more likely to suffer from the disease.2.The main symptoms of patients with hypnotic drug-dependent sleep disorder are difficulty in falling asleep and easy to wake up at night.Mental factors were the main definite precipitating factors,but most patients did not complain about the obvious ones.Most patients have a history of disease;The results of PSQI scale showed that patients with poor sleep quality were in the majority,while the results of ISI scale showed that patients with moderate insomnia were in the majority.HAMA and HAMD showed that patients were often accompanied by different degrees of anxiety and depression.3.The main syndrome types of patients with hypnotic drug-dependent sleep disorder were deficiency of both heart and spleen,yin deficiency and exuberant fire,and liver stagnation and transforming into fire.The main pathogenic factors were qi stagnation,qi deficiency and blood deficiency,and the disease location was mainly in the heart,spleen and liver.Tranquilization is the main treatment principle and method.Teachers mainly treated the disease with dialectical prescriptions of traditional Chinese medicine,and chose Chinese patent medicine or the combination of the two according to the clinical actual situation.Every patient was given sleep health education.Prescription such as Suanzaoren Decoction,Chaihu Shugan Powder,Anmei Dan and Guipi Decoction were commonly used.Semen Ziziphi Spinosae,Poria,Rhizoma Ligustici Chuanxiong,Fructus Schisandrae Chinensis,Rhizoma Acori Tatarinowii,and Radix Salviae Miltiorrhizae are commonly used.The pair with the highest correlation degree of TCM was Radix Salviae Miltiorrhizae and Semen Ziziphi Spinosae.The core drugs were Semen Ziziphi Spinosae,Radix Salviae Miltiorrhizae,Poria,Fructus Schisandrae Chinensis,and Rhizoma Ligustici Chuanxiong.4.The TCM prescription combined with sleep health education in accordance with syndrome differentiation is effective for the treatment of hypnotic drug-dependent sleep disorder. |
