To Summarize Professor Wang Xinchang’s Experience In Traditional Chinese Medicine Differentiation And Treatment Of Systemic Lupus Erythematosus Based On Data Mining Technology

[Objective]Data mining technology was used to analyze the TCM case data of Professor Wang Xinchang treating patients with systemic lupus erythematosus(SLE).To explore the potential law of prescription medication.To explore clinical experience of Professor Wang Xinchang,and share the new diagnosis and treatment ideas.[Method]SLE cases that met the inclusion criteria of this study were collected from Professor Wang Xinchang’s outpatient clinic from January 1,2018 to January 31,2022.The information in the case was sorted out,including patients’ age,gender,duration,symptoms,tongue and pulse,diagnosis and treatment plan,condition change,etc.,and the information was standardized and normalized to form professor Wang Xinchang’s SLE case database.Then,Excel,SPSS,SPSS Modeler and other software were used for frequency analysis,association rule analysis and hierarchical cluster analysis and drug-symptom relationship mining for the data.To summarize the medication rule and clinical experience of Professor Wang Xinchang in the treatment of SLE,and to share new ideas of diagnosis and treatment.[Result]The 116 SLE patients included in the study had a peak age of 30-49 years,and the male to female ratio was about 1:38,indicating that this disease is more likely to occur in women of childbearing age.The top ten common symptoms were tetter,fatigue,joint swelling and pain,dry mouth and eyes,Raynaud’s syndrome,irregular menstruation,hair loss,fever,sleepless and restless.The top five tongue quality frequency were dark red tongue,red tongue,light red tongue,light dark tongue,light white tongue;The order of coating on the tongue is thin,thin white,thick and greasy,little and thin and greasy.The top five common pulse frequency of patients were chord,fine,number,sink and slip.In order of frequency(frequency ≥30),Professor Wang xinchang used Mudanpi,Chishao,Shengdihuang,Baizhu,Qinghao,Huangqi,Yiyiren,Fuling,Danggui,Shengma,Gancao,Chaihu,Zhimu,Jiangbanxia.The top seven commonly used Traditional Chinese medicine categories were tonifying drugs,clearing heat drugs,relieving surface drugs,promoting blood circulation and removing blood stasis drugs,damp-clearing drugs,regulating qi drugs and dispelling wind dampness drugs.The first three of the four qi of Traditional Chinese medicine were flat,cold and warm.The first three ingredients were sweet,bitter and acrid.The top five meridian of TCM were liver meridian,lung meridian,spleen meridian,kidney meridian and stomach meridian.Nine drug pairs were obtained by association analysis of Chinese medicines in prescriptions.According to the order of support degree from high to low,they were Mudanpi--Chishao--Shengdi,BaizhuFuling,Qinghao--Zhimu,Yiyiren--Chixiaodou,Baizhu--Dangshen,Mohanlian--Qiancao,Maidong--Beishaoshen,Chaihu-Zhishi,Chishao--Honghua.Systematic cluster analysis was conducted on the top 69 high-frequency drugs in the prescription,and a total of 8 new prescriptions were extracted.They have the effects of clearing heat and cooling blood,dispelling wind and dehumidification,relieving the exterior syndrome with drugs pungent in flavor and cool in property,nourishing Yin and kidney,nourishing qi and nourishing Yin,nourishing spleen and stomach,nourishing qi and nourishing blood,dissipating qi and removing blood stasis,elevating clear qi to lower turbid qi,evacuating wind heat,dispersing wind chill,regulating and gasifying dampness,invigorating spleen,benefiting water and dampness,tonifying kidney and solidifying essence,and strengthening muscles and bones.Furthermore,the common drugs corresponding to the symptoms were further explored to show professor Wang xinchang’s clinical experience in the treatment of SLE from a multidimensional perspective.[Conclusion]Professor Wang Xinchang believes that SLE is based on Yin deficiency,characterized by heat toxicity,blood stasis and phlegm dampness,and this is a mutual cause and effect.With the development of the disease,there can also be different outcomes and prognosis,such as blocking qi,changing fire to produce wind,wasting qi and blood,Yin and Yang,etc.Therefore,the clinical syndrome is complex and diverse,often cold and heat,mixed with deficiency and reality.Need to address both the symptoms and the root causes.Treatment includes to nourish Yin and clear heat,cool blood and invigorate blood,invigorate spleen and benefit qi,clear damp and promoting diuresis,and to regulate qi,dissipate wind,relieve exterior syndrome,warm Yang.The drugs are combined with cold and temperature.Use sweet medicine to fill deficiency,use bitter medicine to dry and clear heat,and use acrid medicine to disperse wind and warm.Most of them belong to liver meridian,also lung,spleen,kidney and stomach meridian.The drug reaches the blood level.A new view was put forward that "the key disease location lies in the liver,and it is necessary to adjust the liver and restore it".In this study,9 potential drug pairs and 8 potential new prescriptions were found,which could provide new clinical basis and ideas for TCM treatment of SLE.
