Sleep Evaluation And Consistency Of Wearable Devices And VAS Score In Patients With Orthopedic Pain

Objective:To use VAS score and portable wearable device to evaluate the therapeutic effect of traditional Chinese medicine on orthopedic and traumatology disease pain of people with different constitution of traditional Chinese medicine,and to analyze the consistency between night sleep data of patients with different constitution monitored by portable wearable device and VAS score.Methods:from November 2020 to February 2022,107 patients with pain in the outpatient and inpatient department of the second Department of orthopedics and traumatology of the Second affiliated hospital of Zhejiang Chinese Medical University were randomly divided into integrated traditional Chinese and Western medicine treatment group(hereinafter referred to as treatment group)and control group.Patients in the control group were treated with loxoprofen sodium tablets immediately after admission.From the first day of admission,add placebo to loxoprofen sodium tablets for 2 weeks.Treatment group:after admission,patients were treated with loxoprofen sodium tablets and given different treatment schemes according to their main physique:"Qi deficiency syndrome" patients were treated with Guipi Decoction on the basis of loxoprofen sodium tablets from the first day of admission;From the first day of admission,patients with "blood stasis" were treated with alloy Lingzi powder of Bushen Huoxue Decoction on the basis of loxoprofen sodium tablets for 2 weeks.Before and after admission,all patients used the TCM constitution classification judgment table to evaluate the constitution scores of qi deficiency,blood stasis and VAS scores;Wear bracelets from the first day to the third day after admission and the first day before discharge to monitor the sleep time(total sleep time,deep sleep time,light sleep time and number of wakes)every night.The patients were scored by VAS after taking drugs every morning.Results:a total of 110 cases,53 cases in the control group and 57 cases in the treatment group were included.Among them,one patient in the control group fell off due to "halfway out",two patients in the treatment group fell off due to"unfinished scale score" and "halfway out",and finally 52 patients in the control group and 55 patients in the treatment group completed the experiment.There was no significant difference in the abscission rate between the two groups(P>0.05).There was no significant difference in gender,age,diagnosis,score of blood stasis and Qi deficiency and VAS score between the control group and the treatment group(P>0.05).There was no significant difference in VAS scores on admission,the first day,the second day and the third day between the control group and the treatment group(P>0.05);However,there was significant difference in VAS score between the two groups at discharge(P<0.05).The VAS scores of patients with blood stasis and Qi deficiency in the treatment group and the control group were significantly lower than those at admission and discharge(P<0.05).There was no significant difference in total sleep time,light sleep time,deep sleep time and night awakening times between the two groups(P>0.05);There were significant differences in the total sleep time between the two groups on the first day,the second day,the third day and discharge(P<0.05);There was significant difference in total sleep time between the two groups on the second day,the third day and at discharge(P<0.05),and there was significant difference in total sleep time at discharge(P<0.01);There were significant differences in total sleep time,light sleep time,deep sleep time and night awakening times between the two groups(P<0.01).There were significant differences in age and gender between patients with blood stasis and patients with Qi deficiency(P<0.05).There was no significant difference in VAS score,total sleep time,light sleep time and deep sleep time in the treatment group at admission and discharge(P>0.05);There was no significant difference in VAS score,total sleep time,light sleep time and deep sleep time in the control group at admission and discharge(P>0.05).The two methods were consistent in assessing patients’ sleep quality at admission,the first day,the second day and the third day after admission,but there was no consistency at discharge.Conclusion:the sleep quality of patients with different constitution types is improved in varying degrees after using different prescriptions and drugs,but in general,the treatment group of integrated traditional Chinese and Western medicine has more advantages than the control group.The biased constitution score of patients at discharge is lower and the sleep quality is better;Compared with patients with blood stasis,patients with Qi deficiency are older and more women,but there is no significant difference in sleep;In the early stage of pain,the VAS score is consistent with the evaluation of sleep quality by portable wearable devices,but the consistency decreases gradually with the reduction of pain,suggesting that pain is not the main factor affecting sleep when the pain is not obvious.
